Summary
Mark Koernke and Michael Nesser discussed ammunition and powder shortages across the country, advising listeners to collect brass casings and practice marksmanship. They addressed medical preparedness for malnourished FEMA camp detainees, recommending gradual refeeding with clear liquids and electrolytes to prevent fatal complications. The second half featured detailed medical instruction on head injuries, including cerebral contusions, subdural hematomas, and diagnostic procedures, emphasizing that field medics must understand trauma care to keep casualties alive in crisis scenarios where hospital resources may be unavailable.

He took off his three-cornered hat and speaking low to me he said, We've fought a revolution to secure our liberty. We wrote the Constitution as a shield from tyranny. For future generations this legacy we gave. In this the land of the free and home of the brave. The freedoms we secured for you we hoped you'd always keep. The tyrants labored endlessly while your parents were asleep. Your freedom's gone, your courage lost, you're no more than a slave. In this, the land of the free and home of the brave. You buy permits to travel and permits to own a gun. Permits to start a business or to build a place for one. On land that you believe you own, you pay a yearly rent. Although you have no voice in saying how the money's spent. Your children must attend a school that doesn't educate and your Christian values can't be taught according to the state You read about the current news in a regulated press and you pay a tax you do not owe to please the IRS Your money is no longer made of silver nor of gold you trade your wealth for paper so your life can be controlled You pay for crimes that make our nation turn from God and shame You've taken Satan's number. You've traded in your name. You've given government control to those who do you harm so they could burn down churches and seize the family farm. And keep our country deep in debt. Put men of God in jail. Harash your fellow countrymen while corrupted courts prevail. Your public servants don't uphold the solemn oaths they've sworn. And your daughters visit doctors so their children won't be born. Your leaders send artillery and guns to foreign shores and send your sons to slaughter fighting other people's wars. Can you regain the freedoms for which we fought and died? Or don't you have the courage or the faith to stand with pride? And are there no more values for which you'll fight to save? Or do you wish your children to live in fear and be a slave? O sons of the Republic, arise, take a stand, defend the Constitution, the Supreme Law of the land, preserve our great Republic and each God given right, and pray to God, keep the torch of freedom burning bright. As I awoke, he vanished in the mist for whence he came. His words were true, we are not free, but we have ourselves to blame. For even now as tyrants trample each God given right we only watch in tremble too afraid to stand and fight If he stood by your bedside to dream while you were asleep and wondered what remains of the freedoms He'd fought to keep what would be your answer if he called out from the grave is? Distill the land of the free home with the best of them right here on LTR. Well, good afternoon ladies and gentlemen, this is the second hour of the afternoon intelligence report. I'm Mark Kornke. And I'm Michael Nesser. It sounds like George is here. Wow! Okay, we got George, you gotta quiet it down in the background there. One day closer to victory for all of our brothers and sisters both on and behind the lines at occupied territories south, southwest, east, and northeast. Well ladies and gentlemen you are listening to us on Liberty Tree Radio dot 4mg dot com, PBN dot 4mg dot com, and we're on live 365. Then go to Liberty Tree Radio. We're also on AM and FM micro stations, CB Bay stations, and alternate technologies, both east and west of the Mississippi, along with southern and central Alaska, and other places too numerous to mention. Mike, today's date is? 18 November 2008, Tuesday. To...firing one. I'll tell you what it is a beautiful Tuesday here. We're going to be covering medical so I'm going to remind everybody real quick that They better get their pen and paper ready because we are going to be following up and finishing what you started yesterday But before we do that a couple things need to be touched on here And it sounds like we have a call or two. I think we have George waiting in the wings George be patient Oh, okay. Well, that's okay George. You're good. Anyway, a couple things happening right now. Ammunition, as we know, was already clearing the shelves as quick as it's hitting it. Basically like water hitting the skillet, guys. Well, I've been checking on powders and a little conversation we're having off the air between the program. We didn't even touch on it last hour. Powders went the same way almost at the same time. So we need to do powder surveys across the country, but I think we're going to see the same situation. Everybody can see the writing on the wall. They know what's coming. And almost all of the available powders for most of the major carriers are already gone. Now, if you have a favorite watering hole out there, I recommend that you be prepped to take care of that issue and start cleaning the shelf off where you are. because there is going to be a progressive problem in getting both powder, primer and even virgin cases or first generation used cases. So save all your brass. If you're going to go to the range for any kind of training, you don't blast or spray and pray. You should be focusing on rifle and pistol marksmanship. When you fire, you will recover all of your brass. Once you recover your brass, if you can try to keep it marked for the rifle it was used in if you're going to reload it. Why? It's already been chamber sized to your weapon. It'll be much easier to resize when the time comes. Okay, trust us on this. So the little trick there is take along a couple of cleaned out, dried juice jars or one gallon milk jugs. And what you do is mark the brass, in other words mark the rifle, AK-47 number one, AK-47 number two, or whatever number you want to put on there to match the weapon you got. And take the brass, if it's reloadable, box or prime stuff, or even if it's steel case, or whatever you're using. And I'm saying collect it all. We are going to need all of it. We can make it work. We just have to have the basic components first guys, okay? Powder primer shot are all going to be an issue Cases are especially going to be a problem, so we have to be ready for that okay? Anyway, do you want to touch on that we have it sounds like George from Texas George you there? Hello mark. What's going on down that way well anyway? I can tell you a bunch of people so like got their intelligence insulted on my registration sticker, not sticker, inspection sticker, that I got in my car changed over. It says, check the date, love your state. Then they had this commercial scenario, check the date, love your state. When you register your car, it goes towards building roads, bridges, highways, whatever. And it's like, how stupid do you think we are? Our gas tax is paid for that. Yeah, well that's what they're supposed to I think what they're saying trying to tell you know in an insultive way is the money you're spending in the other direction is probably being stolen for the naff to superhighway That's quite possible and It also too. It's more like then it dinner passing all this stuff in the paper saying Lottery sales are down though the schools might fail if we don't play the lottery. I'm like get out of here I mean like property taxes pay for that You know what, do I hear the wave of lift? Hold on here, can you hear me? What is that guys? The world's smallest violin. I feel so bad. You know that racket with the lottery? We got hit with the same scam up here in Michigan guys. And you know when they pushed that years ago they said, oh for as long as the sun shines and the waters flow. If we have the lottery your taxes will go down and you won't even have to pay property taxes for your schools because the lottery will finance it all. And you know what happened? Well first the Harpies, they were already hooked to your neck with the property taxes and everything. And then they were in a confused state for only a few moments guys because they looked at your throat and they looked at this puddle of, you know, this plasma bag of other money floating down from above. And you know what they did? They hooked it up to your body and they just kept sucking anyway. They just kept sucking. In fact, another one hooked to your throat on the other side because they felt they could get away with twice as much. And so they didn't make schools more efficient. They became more bloated and grossly overpopulated with bureaucrats. And now the rest of the story can be seen because look at the poor quality product they're putting out kids. And again, for the amount of time that the kids are spending in school, what are you getting? Okay, so I don't think we've got anything good out of it basically just a reverse a whole bunch of Soviet socialists got special post made example is here in Dexter Guys there's a school that's been closed down in my lifetime in Dexter and it's not like our population It did get bigger, but it was purely a lack of efficient planning and use of space We have a high school we shut down because they built a new high school while I was away. And what's really cute is they turned half of the classrooms in the old high school into individual offices for single people. Whole classrooms. a whole classroom for a single bureaucrat, a single paper pusher. Do you know that? And then of course put temporary, think about this one, took school rooms away from the students and then and shuttled in a whole bunch of temporary classrooms because they had to show that they were overcrowded so they could try and demand more tax money for more schools. Yeah, here's what your lottery did for you. Absolutely nothing. And it is true, you know what George, I'd have to agree. Chances are, people can't pay the gas for the car, they're not going to be running out and jumping on a lottery ticket. You know what I mean? Maybe if they collect enough beer cans, or pop cans, or pop bottles, but that's about it. Anyway, what else you got for us? Go ahead. Yes, this one's for Mark, it might be for you. I was sort of like thinking though, if we liberate a FEMA camp and it's sort of like everybody's skin and bones like in Auschwitz, or Bergen-Belsen, what can you give them? You can't give them big portions. You want to give them a portion of a meal that gives them the most needed nutrients available, needed right now, without having to give them sick or anything. Well, actually, your best bet would be in a situation like that, would be to produce something kind of like what would be called a neutral loaf. What you do is literally you take all the food stuff you have, break out the food blenders in whatever kitchens or mobile kitchen you can create, and you convert it over into a bread stuff that basically is vegetable, meat, potatoes, tomatoes, I don't care what it is. And what you do is you actually limit physically how much they can access because it's true. When you have If you've been a prisoner who's been in like the hole or in solitary confinement, and I've seen this quite a bit, you're in a situation where your diet's been limited and you have only a certain number of calories you're going to take in. If they decide that you're not a nice prisoner, they put you on what's called a neutral loaf. Literally, what I described is what it is. It's just a pile of slug of food that they put together with a certain amount of bread stuff. And that's what you get fed for so many days on end. Weeks on end or months on end, depending on how they feel about you. Now with prisoners, it'd be one thing. That's a torture. But when you're dealing with individuals who are prisoners you're liberating, you can't let them just gorge because literally the system will shut down. They will blow up on you. So restricting the intake but making sure that it has a wide range of nutrients would be the key solution and the NutriLoaf package would probably be your best choice. That or feeding them the guards but I don't think I'd do that to poor people. We'd avoid that. So might have the guards eating each other but that'll be later. Anyway, tell you what, we're gonna go to break. Everybody stay right where you are. This is the Intel report live. It is the second hour. It's Tuesday. Mention Liberty Tree Radio for your listeners discount or just call us at 734-340-7285-734-340-7285. Well you know some of them did some of these. And we're coming back. Ladies and gentlemen, this is the Intel Report. You got Mike and Mark. We're back. And Mike, you wanted to follow up because, again, we're looking at how to deal with this problem. We got a lot of casualties. They've been behind the wire. They're malnourished. What do we do? Okay, I would definitely stick with our liquid diet at first, what we refer to as a clear liquid. not necessarily clear as in water, but something that you can see through, whether it's a PD-alite type of thing or Gatorade or Powerade or something like that. I'd also continue with the diet that they're already on. If it's just some thin soup, potato type soup with maybe some onions in it, and then we'd increase the diet that they're already on, but I would stick to the liquids at first. first and then start building them back up. What happens is that people can't handle some of these solid foods and they're going to start vomiting and if they don't collapse and die at your feet, then it's a lot easier to clear somebody's airway and get them breathing again if all you're doing is trying to clean out liquids. But if you've got chunks down there, it's going to be much harder. So I would slowly increase the diet on what they have already been fed and then start increasing the electrolytes through some of your energy sports type drinks. And then after they've stabilized that, then start working in some a little bit thicker type of stuff as they go on from there. But you're going to have to really be careful about overwhelming them. People are always real generous, and they'll want to reach into their pack and give them an MRE. And that's the wrong thing that you're going to have to do. And I think that one of the big things here again is when you have casualties like that, the issue is going to be discipline because some people will or have the working knowledge to understand the threat to their person, but others, hey guys, the scent of food is enough to drive you crazy under the right conditions if you've already been starved. That is something, if you have not dealt with it before, that's one of the reasons for the caregivers or medical support personnel or anybody who comes in that is moving into a situation like this. you have to start by you have to have started regiment right away doesn't mean you're trying to abuse the prisoners just reverse you're trying to get you you obviously are going to try to move them out of the area of operation as quick as you can get them off site so they're not at risk uh... one of the things to consider is pima camps it's just like a policy that uh... well okay let's back up you're dealing with socialists okay socialist hate people They do not like people. It's a socialist that proposed murdering 80% of the population of the planet down in Texas, guys. It's an absolute nutcase socialist that you're going to be dealing with, Soviet socialist, that is going to be operating here. In the Korean War, the policy of the Koreans was, if it looked like American POWs were going to be repatriated, they executed them en masse. Now this isn't talked about very much because they want to do the warm fuzzy rub your tummy nonsense of course with China. But I suggest that you go back and look at the history of several of the operations that took place. One of them was a special mission to rescue POWs. And the Koreans and the Chinese literally put the men down in the ditches or in some cases had them dig holes quick. But most of them they just put them in the ditches and they blew their brains out or shot them in the back or whatever. Now some men survived that. Now do you think that these shysters don't have a consideration? Okay, think about this. You got all these nutcases that have been overseas, that are in Iraq, that are going to be brought back, that are mercenaries. I'm not talking about our troops, though. There's a bunch of troops that are in the same boat because there are certain characters that like what they're doing. They've been over there kicking doors in, they've been over there putting guns to kids' heads, to women and children, etc., etc. They're the ones that have been ransacked in places, rape, kill, pillage, and burn. Okay. Those people are going to be back over here, but they're going to be putting a little spiffy black uniforms to be used on the American population. Do you think that once they are caught, or if they suspect or fear that they're going to be caught with a detention camp like this, that they aren't going to look at the possibility of trying to destroy it physically? So there are a couple of problems you have to look at. If you were to liberate quickly and if you do liberate a FEMA camp that has been filled up and has been used for a while, it's going to be basically a death camp. Okay? The bad guys are probably going to have a standby order to dispose of the site. which means that on the one hand you're going to want to try to service, you know, like take care of these people as quick as you can, but you also have to have a plan to start moving them off-site as quickly as you possibly can and dispersing them for survival purposes. So there's some real control problems here the bad guys then these are the nutcases that want to murder the socialists that are coming in the enemy you're facing Has already said they want to murder pretty much in fact eight out of ten people on this planet I'm gonna ask all of you to go into the bathroom or go into the kitchen and look in that mirror that you've got set up on the wall and Ask yourself if you think you're one of the keepers And I guarantee that as far as the bad guys are concerned, if you're willing to do that and think you're not, okay? So that's one of the things George, and we're going to let you go here a second here. I want to get Mike on track with what he's going to cover today. But I hope that answers some of your questions, some of the things we have to take into consideration. If we find a FEMA camp, guys, and there'll be more than one, there's going to be bunches of these. The other half of it is arming up as many people as you can. I don't care if all you get had were 22s to hand out. You get these people motivated and understand that there's no going back. Those people are going to be the raw fighters because they'll understand they've already seen it, they've experienced it, some of them aren't going to be physically capable, but they can recover. But the idea is those people are going to realize they've got nothing to lose and everything to gain by plugging the new world order and wiping the buggers out. So there's a number of reasons for wanting those people alive. We need the witnesses. We're going to help our people anyway. I mean, I'm going on a whole long checklist for that issue. But we must be prepared and understand that our enemy is going to... they'd frag a whole city. Okay, here's this list. Let's think about what everybody's been talking about for the last year and a half, right Mike? We've talked about this on the air how many times? If they're willing to frag a whole city, Mike, you think they're willing to frag a FEMA camp? in a heartbeat. uh... you know just was put in the uniform as a military you know as a military soldier and these people that are being recruited as as interior police shock troops i have no i have no compunction uh... and no love will be lost for any of these people think they're going to be shocked through slash secret police wannabes Their days are numbered as far as that goes because I understand that if they win they would murder you George, your family, my family, anybody's family in a heartbeat and laugh about it back in the locker room. That is the one key mistake that is a problem that has been made over and over again. It's like when those bat faggots were being shot up at Waco. They attacked three times. The Davidians tried to talk every step of the way. That could have been dealt with much more rapidly, but the nutcases were in the bloodlust to murder, murder, and murder. The only reason they wanted to talk and when they realized they couldn't yuck it up back at the locker room and you'll get away with it is when they were down to 40 rounds or less of ammunition. That's what was left between all of them and they finally had to talk because their arse was grass and the lawnmower was going through it. Now that's the thing to remember about these creatures. It'll be funny. When they negotiate or they talk and they set you up, they'll be laughing their hind end off later and you'll high five it at the whole nine yards. So why am I going to cry about them? Why am I going to even, in fact, well of course I am because I do have an obligation. If I'm a militia commander and I'm in the field, I have an obligation to meet certain requirements under the rules of war and I will. Our side will. Their side won't. I will swear to you and make no mistake about it, what have they already shown us? The word torture is on the lips of the American people. Never in the history of this nation did you hear of that nonsense until the kosher mafia got into the position they're in now. Torture is on the lips of the American people and was considered, oh well, we can live with that or let that else acceptable. Think about that. Look about how far around the corner the nutcases are on the other side and here's the thing. The buggers that murdered at Waco. are the ones being put in charge now. Think about that. The management is back. The same bunch. Not kind of, sort of. Look at all the people assigned. Every one of those son of a buggers is from the same pack of thieves. Change my arse. It's the same old gang from the same old song sheet. So you tell me what their agenda is. I know this gang. We know their bag of tricks and we know what their agenda is. So ain't nobody here better say they got caught flat-footed or that they're in crisis and surprised because they can't say that anymore. That diversion we've been trying to ignore is the attack we've all been expecting. So here it comes. Okay? Anyway, George, anything else we're going to let you go because we're at the bottom of the hour. How's that sound? Well, I'll just say one thing, that on the rising gun sales, I just hope that's a deflection of the Obama agenda. Well, it's more than just Osama bin Laden. Like I said, he's just a meat puppet. Look at all the faces that have come up now that have been assigned. And he didn't pick them. You know what? Keep that in mind. He didn't pick them. This is all CFR doing this, guys. Appreciate that, George. We'll stop them, but we've got to work together. Thank you. God bless my friend. God bless. And I'll tell you, Mike, we're at the bottom of the hour. Everybody get your pen and paper ready. Mike's taken over here because we have a lot to cover today still. This is Como and Medical Tuesday, back in three on LTR. Yeah, you know Tom. I'm gonna ask you again. Would you put the glasses on? No one can take a second. Just try the glasses. They're not expensive sunglasses. Just try them. Well, you know, like, are they like polarized or something? Well, what do you care? They're free. I'm giving them to you. Just put the glasses on. We're sitting here relaxing, having a cup of coffee. We're in a coffee shop. Everything's cool. Go ahead. Just put the glasses on. I don't know, Mark. Maybe I don't want to know. You know? This is pretty freaky stuff you're talking about. Alright, you're my friend and I trust you. I'm gonna put these glasses on. I'm gonna put the... Give me a hand with those glasses. Here, okay. Okay, not a problem. Put them on. Why are you... I know Tom, in fact, look it. There's one on that woman's shoulder too. It looks like kind of a giant slug with like an octopussy kind of mouth. It's just crazy. It looks like it's got... The way it's born in, it's kind of scary, isn't it? It looks like a ball worm. Not at all. Does anybody else know about that? Tom a lot of us do we've all been paying attention for a long time. You'll notice look on the one. How did you find out about this? Well Tom Tom it okay relax Tom remember We're all thinking we all know what's going on I've been watching this all the while you and I've been sitting here as they've been coming and going but you know the way to find out how I found out and got the glasses I went to live 365 And then I tuned into Liberty Tree Radio. In fact, I found there are other places like PBN.4m2.com. I'm really worried about this. I'm really worried about this because I... Oh, I know Tom. Tom, look at that one over there. The one tentacle says Neocon and the other tentacle says Liberal and they're wrapped right around that wall. Isn't it? It's hideous, I know Tom. But after a while you get used to it. I've been wearing these glasses for a long time and you can see everything now. You notice, look at these other people. It's almost like they're in a soup. It is definitely not Rose Stenkel. But Tom Tom once you put the glasses on and now that you know you can never go back to sleep I know okay, Tom. I'll tell you what we go home I want you to get on the computer and watch you go to live 365 Punch in Liberty Tree radio then you can go or you can go to Liberty tree radio dot 4mg.com or you can go to PBN dot 4mg dot com, but you know what Tom Like I said, calm down. Start to get focused. Oh, the closet monster lives! Oh yes, in fact, look at that one over there. Oh man, that guy looks like death warmed over, doesn't he? His eyes are starting to roll back. If you take the glasses off for a second, Tom, you notice something? They look perfectly normal. Put the glasses on and keep them on because you never know what's coming up behind you. Let me check... Oh my god! Oh my god! Doc, there's one! He's trying to get hold of your throat! Tom! Okay, Doc, okay, wait a minute. I got him, I got him, I got him, it's okay, I got him! Oh, I'm getting him, it's one of those door things! Okay, you're safe now, Tom, we gotta get out of here, let's go! Okay. You're it. Mike, you're it. There's always going to be a variable amount of freshly clotted blood in the subdural space. That's going to be between the inside the skull, but depending on which layer, and then between there and the brain. Now it's going to be accompanied by a severe contusion of the underlying brain. And then usually it's the severely contused brain that is responsible for the patient's compromised condition and not the clot in the subdural space. And this type of a hematoma is fatal in up to 95% of the patients. The next one we have is the subacute subdural hematoma. Now that implies that there's going to be large amounts of recently clotted blood in the subdural space with either no contusion of the underlying brain or at most just a minor part. You might have small patches of subarachnoid blood in there. Now, the quad is responsible for the neurological deficit and its timely removal almost invariably results in a real good recovery. The next one is going to be a chronic subdural hematoma. Now, we're going to mention this in connection with the acute head injuries because although present in asymptomatic for months, it can present very acutely, real rapidly, and the fluid similar in appearance to crankcase oil is going to be located within the brain, or well between the brain and the skull, and in turn it's within the subdural space. Generally the treatment is going to be some burr holes, and followed by drainage of the fluid in the irrigation of that cavity and it's almost always results in a real good cure. The last one is going to be acute subdural groma. Now the origin of the blood tinge fluid present in the acute subdural hygromas and the reasons why small amounts cause such severe neurological deficits are really not known. Now, the fluid's really indistinguishable from cerebrospinal fluid, and then the drainage of the fluid results in a real impressive improvement of the patient's condition. So, that ends that there, and I'll go over them, not the whole definition, but just the word itself. So, number one, we have cerebral. Number two, cerebral contusion. Number three is a linear fracture. Number four is a depressed number five is a basilar fracture and that's spelled B-A-S-I-L-A-R. The next one is intracerebral hematoma. Number seven is epidural hematoma. Number eight is full intracranial aneurysm. Number nine, acute subdural hematoma, 10, subacute subdural hematoma, 11 chronic subdural hematoma, and 12 is acute subdural hygroma, and that's spelled H-Y-G-R-O-M-A. The next block here is going to be the clinical evaluation. Now, the signs of progressive increases in your intercranial pressure are going to be the progressive declining in the level of the consciousness. Number two is going to be the change in the size and the symmetry of the pupils and their reaction to light. Number three is going to be a slow, steady slowing of the pulse. And number four is going to be irregularities of respiration. Now, the presence of one or more of these signs is an adequate justification for bilateral carotid angiography. Again, we're covering over these things. I understand a lot of people don't have these types of equipment, but you want to check out the carotid pulses to make sure that they're equal on both sides. Then the cranial exploration is indicated if there are any signs of a clot or any lobar contusions are found. If any one of the lobes of the brain are found that they have a contusion, then there's going to be a real good sign that they need to have a cranial exploration. Now, the next one is going to be adults. We're going to break it down here a little bit on what to do and when. Now if an adult, if they have a previous loss of consciousness and now they're alert, and if there's a definite history of a head injury and a loss of consciousness, the patient should be admitted to the hospital for observation. Now if the patient is alerted at the time of the examination, it would be nice if you could get a skull x-ray, anterior and posterior, right and left lateral. of views, but the lateral views are best done across the table to avoid having to turn the head when the status of the injury of the neck is not clear. Now, if there's no fracture, it's seen 24 hours of observation are probably going to be adequate. Now, if there is a linear fracture over the top of the skull, the convexity of the skull, and especially if it where the going now, the observation should be prolonged. Now if the level of consciousness starts to decline, then the examination and it should be done to diagnose it, that any hematoma, then this is going to be a prelude to any areas. There's going to be a longer observation that's going to be necessary. Angiography and operation are indicated if the patient becomes un-alertier things. going to be a possibility then you're going to have a displacement of the meningeal or your cerebral vessel in the angiogram and they're going to lead again to the exploratory craniotomy. Now if the X is 5 millimeters or more it should be elevated as an elective ability of an air embolism from the opening of the fire, the use of what we call during operation. Trandellanburg is kind of a slow, is lower than their feet. of an air emboli coming in there. There's a skull in the depressed frag be taken directly to an operating room. Lay the lacerate in an emergency room or where you are and the wound should be irrigated, the edges of it are going to be debrided and any depressed fragments removed in this cart and the scalp is going to be suit sizable. It's going to be over any vital areas and a neoplasty should be prayed or And then bear in mind that some neurosurgeons, they're going to want to wish to wash the bone, place them using some antibiotic cover. A lot of times it's going to depend on the size amount of bone fragments, the scalp and everything should be fine. But a large amount, then this is a pretty serious operation. Usually what they'll do is drill in a triangular position and then they'll cut holes and lift that hole and they can replace that. there and then that will grow back usually. If there is a linear fracture, you want to irrigate and debride the scalp wound and repair the laceration. It is pretty straight. Now in adults with a previous loss of consciousness, now they are irritable or confused. If there is a definite history of a head injury and loss of consciousness in a patient who is now real irritable or confused or without any neurological deficits, the patient for several days or a week and the loss of concussion, the patient becomes more alert, type of neurological changes develop, you're going to consider have an angiography or the x-ray and to determine if there is a collage within the edema. Now on the possibility of a lumbar puncture, where they put the needle down towards the lower back, a lumbar puncture should be advised by some people. Now it's probably safe if no fluid is allowed to escape when the pressure is over 200 millimeters. Now that's if they're laying on their side and they're all And it may be useful if the patient is not too restless to prevent a proper estimation of the pressure. It's going to imply that there's a cerebral contusion, or a hemotoma. So if you do a lumbar puncture and the CFS could have, even if there's a subdural hemotoma, if there's increased pressure, it may be present with a contusion, or with a concussion or a contusion, and sometimes with a hemotoma. So you can do the lumbar punctures. there's no real, don't be misled that if you get clear CSF, it just hasn't migrated down there. Now the presence or presence of a fracture does not correlate with your CSF pressure in the CSF. If they're only responsive to PIMULI, take and only if they can be done conveniently and quickly and if there's possibility then you can repeat them. Shift in the midlines, looking in there, everybody's seen of a brain or a skull from the top and you've got the left hemisphere and you've got the main blood vessels running down the center. So if you see something where things are being pushed or the other and it would be nice if you could get a cerebral angiography, maybe reveal a hematoma or indicate a contusion to the brain. The state of consciousness is rapidly deteriorating, become unequal or if the pulse is going to have to go for an explosion. angiography and urgent procedures. So if patients can deadly deteriorate, the pupils become unequal or not worry about getting raised or anything else like that. Now if there's a contusion of the temporal lobe, the cerebral angiography are going to show an elevation in the middle cerebral artery in the anterior and the posterior x-rays, and sometimes also anterior cerebral artery. and the treatment is usually a resection of the tiptoe, acute hematoma, impossible to distinguish from an epidural hematoma. Evidence of medial displace of the middle arteries to indicate one of the diagnosis. This is are you gonna go to the bur holes or a craniotomy with removal of the fresh acute. So hematoma is present. Then the angiography is gonna show irregular outlining of the deficit over the surface of the brain with evidence and then there can be also an underlying or a contralateral hemisphere on that the other day. Somebody hits the steering wheel or hits the always going to be in the front of the brain, itty to the back of the brain. If somebody's up on the ladder, they fall off, they get the ability that you have a bruise on the right side of the brain. So to lock in into that. Now touch about a little bit about lumbar puncture, painful elevated over 200 in the lateral decubitus position. Again, they're on their side and the dema, contusion, or hematoma is probable and an angiography or an operation may be urgently needed. Now, if the paranormal and the hematoma is less likely and then you're just going to be observing the patient for changes in your clinical, ensure it's below 50. is there's a herniation of the brain as a tentoral notch or the human magnum brain and then it goes through the hole down into the magnum. Now if the pupils are not dilated and fixed and the immediate exploration is indicated, the magnosis is diluted, dilated and fixed. The cases a little bit about staring may help fight edema, actually when there's no operable lesion is going to be found. Now our loading dose of 36 Again, B, T, H, and E. That's going to be a loading dose of 36 milligrams, and then you're going to follow just about finished here. It's under two years of age. If they're unconscious, it's a time of examination, and it's really extremely rare in infants. Now, neurological deficits or changes in the pupil size, blood loss may occur, and if you've got the bandy arteries, could possibly be bleeding. Now lost blood, the man covered the anti-seizure medication and the helping of the, the down of the swelling of the brain is going to be, they should be used to help reduce the cerebroledema. And if you suspect that there's any intracranial injuries, only just the schema-toma should not be tapped immediately since it almost always will reabsorb within about a two week period. in the lateral. Well, linear fracture committed to the hospital and it observed for about 48 hours. They should be followed as an outpatient for the oral hematoma and the subsequent skull trays are going to be needed to ensure any absence of the enlarging of the fracture. At millimeters, the child should be admitted to the hospital, seems to be elevated as an elective procedure. And then there's wide fractured edges. The child be observed in the hospital for any accumulation of hematoma. Again, so you've got to make sure that there's none of these fragments of the brain that have been lacerated in any of these blood. Clear history of any trauma, but the evidence of both of the sutures, the suture, when we call about sutures of the skin, that's where we're sewing it together. Children are born, the bones are all separate, permit to be squeezed through the birth canal. And then as the child grows, then these bones together, and those are the lines that run behind where the forehead is from ear to ear, and then down the center of the skull, the suture line. Suture, if they have vomiting and there's a failure to thrive of convulsions or any irritable child, you're going to have to definitely consider the possibility of a chronic subdural hematoma. the future. No, I just want to real before we go any further because then we're gonna run out of time here a second real quick Well, I want to back up for a moment here and talking about the head injuries like this one thing people are thinking They're not gonna mess with that now. There's one thing we don't do We're not gonna be debriding the wound the way we would other say chest cavity or leg injuries only in that we're not gonna be doing a lot of you know probing and such okay to get that you know that has to be qualified. What Mike's talking... Opioid dependence is a medical condition many people struggle to overcome that too many try to handle on their own alone but they don't have to and neither do you. Treatment is available in the privacy of a physician's office. Click on the banner and visit turn2help.com. Live 365. This time now especially with special warfare says slash special forces medical operations To have to keep the guy alive for anywhere from five to seven days with only the support you're carrying on your back Now for all of you listening right now as field medics or even people that are thinking about having to do medical support You need to think that way and longer Because, consider this, let's look at one of the scenarios guys. One of the cities gets fragged by the Israelis combined with the Fed government, wanted to create a condition like we've all been talking about. How many trauma casualties do you think are going to be absorbed into the system? How quickly? Now, do you think that you're going to get any kind of priority at a hospital if something happens? So what do you think your chances of survival are if you know how to do most of what we're talking about first not only for yourself or for just the family but for other people that you're assisting in the unit or that are part of your community? You see how critical this is? I want to qualify that real quick, Mike, because you see people are thinking, wow, the head's going to be a little, that's radical. Hey, guys, it's either something or absolutely nothing, because you see what'll happen is in a lot of cases with triage, with an optimal crisis, like say a thumper, like a nuke, there's going to be a guy looking at somebody that's a casualty, like you're describing, Mike, and they're going to say, well, normally I could fix that, but I don't have the time. Put them over there in the death ward. Okay, keep that in mind. Our mission is to keep our people alive. To do it, we have to have this working knowledge in place. And that's how it ties in. Mike? Absolutely. You know, one quick thing here. I was just going to say that we have to understand that you pointed out before that in World War II they took the medics and they started teaching them how to do appendix. I'm not expecting everybody to be competent in doing brain surgery, but at least you'll have an understanding of what's going to be required and you can pitch in and help. I don't want you having your hands in your pockets going, hey, you couldn't do that. One of the key parts about this, guys, I know we're right at the top of the hour, but one of the things to remember, any aid you render is going to bring that patient closer to the survival line. That's the critical thing here. That's why we I appreciate what you're doing Mike and again I want everybody to think that way too. Don't forget.
