Hopefully we cleaned them out or helped to finish up the difference but as we know everybody is pretty well picking up anything and everything as long as it's Available, but let's let's point something out here in many calibers Everybody is not only running out, but simply can't find anymore, okay? We warned everybody about this before I will point out that if you are thinking about Getting a weapon the first thing that I would do is when you find the ammunition you buy it first By the ammunition first then seek out the firearm, okay? Now the one reason I had I've got more than Mike I've got a few people on the same kind of mission here as we're trying to find out if there any pockets of a K 74 is left out there at all route the reason I bring this up is because There's still a lot of five four or five Russian out there that is cheap by comparison to everything else and the 545 by 39, it's equal to 223. It'll work just fine guys. It's an AK action, AK ammunition. Plus you're looking at AK mags. Should be pretty straightforward, but what's happening is everybody's doing the same thing we're talking about. They've gone out. They said does it look like an AK? Yes it does. Does it work like an AK? Yes it does. Well then it's sold. Now you're gonna have to search around in the retail and probably to find what's left laying out there But we're gonna find out what sOG's situation is and a few of the others here by weapons Wednesday So we'll let you know and even then it's almost a daily thing right now Mike Every company we've been dealing with a few days ago they had 303 left in JMG sales and In the last couple of days as as more of reality. I think is setting in people are realizing oh my goodness I need that oh my goodness I need that oh my goodness I need that too oh my goodness I need all of that and more I would go so far as to say right now if you have money to invest invest it in ammo no matter what it is even if you don't have something that shoots it That's another thing to consider is if you're something you have somebody that's nearby where you can access a Pocket of ammunition that might be available It's a little more reasonably priced and you can access and you know limited amounts say a little bit here a little bit there and Continually invest you are not going to be losing a penny at all. Look at it as the other precious metals. Okay And the reason I bring that up is because we've been surveying the whole of the US and the picture is as expected. It's not unpretty or ugly. I'm not going to say that. I'm going to do something I've done many times in the last couple weeks. We told you so. Mike, how long have you been doing this program? A little over a year now. And what have we been talking about all the while we've been doing this program? Get out there and get your stuff while you can because one of these days it's going to dry up. And the reason is exactly what you're seeing right now. We're hitting that next wave. In fact, next after this comes undertow by the way. But we're hitting that wave. It's already peaked with the election date. Just after the election date, all of a sudden reality started setting and it's progressively now the ones that were probably stunned for a little bit. are finally starting to move and get in motion and so that's part of the next cascade of waves we're going to be seeing over and over again and I will say this this is holiday season coming up you know what you give your your favorite boyfriend or your favorite you know husband or your favorite grandpa you give him more ammo you give him magazines and ammunition and equipment that fits his weapon so that he's happy he's even happier still If you've got a youth that is in training and you want to try and develop their skills, this is where you get them their Christmas 22 or their Christmas 12 gauge or their Christmas 20 gauge. That's when I got my first rifle. That's when I got my first main battle rifle, my deer rifle, which was a 1903 Springfield by the way. And for that reason, once again, I will remind you that an excellent opportunity right now. Find what you can. Now if you can't find an AK or AR, well that doesn't surprise me at all. So now you've got to go right down the shopping list of what else might be available out there that's a combination of reasonably priced and ammunition accessible. If you can get the mags and the weapon but you can't get the ammunition there's a bit of a problem there and you're gonna have to shop around. Now another thing real quick Mike that I noticed and this is surveying several sites a lot of the calibers that are the, not the oddball but they're like the Leeside calibers 303 British has pretty well been swept up by everybody. Why? Because there were a whole bunch of British 303s that showed up in the 90s a lot of them, millions of them. Another thing was what I thought was surprising is 765 Argentine completely bought out 300 win mag, completely bought out guys, even the $70-$80 a box ammunition is gone. Anybody and everybody that's got a 300 win is picking up all that they can and buying more. So that again is an example of the path is not as direct as you might think. In other words, you'd expect certain things to go in a certain direction but they're not. The MBRs are making a major dent in the ammunition supply, the MBR operators. and that's where you're seeing a lot of these unique calibers showing up and being made to disappear. This is a good thing. We like this. Okay. Now, down in your stretch of the woods, let's see, you had a chance to talk to them a little bit. What was the response as far as what they've been seeing business-wise and what was going on down there? Well, my impression, I called up a couple of times and the line was busy. I kept calling back. A woman answered the phone. I said that I had some questions about availability of firearms and ammunition. She put me on hold for about 10 minutes. So that told me right there that they're very busy right there. So when I asked a gentleman if they had any AK-74s and magazines, we had no AKs, period. I guess I kind of answered that. Then I asked him about the 303 British ammo and he said none. Then I had asked him, I said, I guess you guys are kind of busy right now. He was like, oh yeah, sales are real busy, brisk, going through the roof. He said also since the election. wasn't this going on before the election for about two months? And he said, pardon me, and I repeated my question and he says, oh yes, it was going then but no way near what it's going now. And he said, you know, I asked him also, is this has something to do with, the word on the street is that there is a soft ban by the Bush administration against import and he said that has something to do with it. really wouldn't confirm or deny or expound on it but pitted on the uncertain election and now that the election has been clear everything's just kind of just as a side note and I guess this came to the weeb and geez set in my hand it's January, February, March, 2008 and they have the Romanian AK-74 Rhapsody magazine with $349.95. 350 bucks you could have picked one up a year ago. This is something that's why I always bring up the issue of burn casualties Because burns can happen in so many different ways that people don't think about And it's one of the most difficult of all the injuries really to deal with for what it is. I mean we can have internal injuries that are easily handled in burns simply because of the nature of the environment around you. People get dirty, infections set in, it's a constant care issue with burns that need to be taken care of now, you can't wait till later. The other thing about that is the immediate response with burns is more critical than probably any other injury of its type because of that issue. The sooner we can treat, the faster and more efficiently we treat, the less injury in depth we're going to see and the less pain the patient's going to go through. I will say this, if you have not dealt with a burn casualty, then the one thing I'm going to remind you of is you better be ready for dealing with pain. Not yours you're the caregiver, but you better be ready to be dealing with a patient who's going to have to be dealing with pain and It's something that a lot it's something you don't show for a reason and it's something that no we don't need everybody No, it doesn't need to experience it, but you need to be girded for this I think that's another part Mike that we need to touch on even today is if you're a caregiver If you're a responder, then you've got to be ready mindset wise correct Oh, all the time, but not obsessed. No, no, but you know what I mean. In other words, no, but there's a, you see, this gets into something. I've watched this happen over and over again. A lot of people, you know, somebody has to take charge. There's a lot of situations where you have a lot of healthy people standing around with their thumb up their butt, you know what I mean? And when we're looking at a lot of casualties, one of the things that people are not going to be allowed to do is sit there and vacillate and agonize and whine or lament about how terrible the situation is. There are two things you can do to deal with that. Number one is grab them by the hair, so to speak, and give them a job. Grab them by the ear, grab them by the short hairs, whatever term you want to use. And you give them a task and you tell them, good, you know what, this is really bad, help to fix it. Number one, but the first element of that is the individual who is actually the the responder is going to have to have a specific mindset to overcome what is basically an issue dealing with the casualties pain. This is something that's hard. It's hard for some people. They are awfully callous. I always heard that term at different times. It's like no, I am completely cognizant of the fact that to keep that person alive, I'm going to have to deal with their pain in a different way. Okay, I can be sympathetic, but I cannot be sympathetic to the point where it becomes a detriment to my being able to perform. And that's something that again is the drama or the soap opera version of what they want as opposed to the reality of having to deal with trauma casualties of this type, especially Battlefield situation. There are some good write-ups in that, you know, Black Hawk Down was a propaganda piece with the movie, okay, and it was intentional. That's the only way the army would allow it to be done. The only way they'd provide support is if they made it into the win-win, it was a wonderful thing. What everybody needs to do is read the book Black Hawk Down. because it gives a more realistic impression, you know, direction for what kind of impression the authors were trying to generate. But one of the things that they did tie into that was the medical responders and the fact that the corpsman who was on the ground, the men who were actually dealing with the casualties in the column, what they had to do in the field in that situation. And in some cases they obviously still didn't save the men, but they did phenomenal work considering that in many cases almost all of them were wounded. So, again, it's mindset. When you're dealing with a casualty, you do to a degree detach yourself from what people would consider to be the norm simply because you have to stay focused. You've got to be able to commit to do the task, get the job done. So we want that to be part of what you are going to be willing to embrace when the time comes to deal with, not one, casually. You may be dealing with a whole slew of people. people are looking for guidance, they're looking for direction to a degree. You have to be stone hard when that happens. You gotta be able to stay focused and keep everybody else online too. And it's something that is difficult for some people to appreciate it until they face something like that. When you have 10, 20, 30, 40, 50, 60, 70, 100 casualties all at once, and that can happen easily, we've talked about this many times, Someone's going to have to get their act together or somebody's going to suffer accordingly. So that's one of the reasons we bring up the medical support issues the way that we do on the program here guys. We want to build up the base. We need the supply and support system in place to work with it to support it. But we need the people. We need the manpower already up and online. We can't produce them later and get the results we want. We have to be doing it now. It has to be at this point in time while you do have to a degree the luxury of being able to sit back and look at the situation and develop what's necessary through checklists and because of educational processes that have been developed. That way when the time comes and the old feces hits the oscillating device, it's going to be second nature. It's automatic. It's part of the programming. It's part of your educational process and it plugs in. That's really how it works with Corman, correct? Absolutely. You know, one of my best phrases was, this is going to hurt you a lot more than it's going to hurt me. Yep, exactly. And you know, it's not to be callous or anything, but I don't know, sometimes when you're dealing with situations, you know, you have to have a little levity in the whole situation. You can't, I don't know, I guess maybe it's an escape or a defense mechanism. As to where you can sympathize and empathize with the patient. I've been injured myself before and been on the receiving list. So I know exactly what's expected. You know it's going to hurt, but you're going to do it as quick as you can to reduce the pain. It's pretty interesting stuff, but next month is crunch month. For every ounce of silver that is known above ground, there are six claims to it, which means they're going to want delivery next month. Wait a minute, there's a bit of a problem with that. Isn't that what we've been saying all along about this, Al? Oh, absolutely. I was teasing with Don the other day. Now, this next block, we're still going to be following up on our resuscitation from shock and trauma. This next block of instruction is going to deal with head and cervical injuries. It's going to be a fairly long block. We're most likely going to have to divide it up in a couple of days here. So just as a heads up. Now, the most important observation to be made in a patient with a head injury is the state of consciousness since the injury. You want to record the present level of consciousness, preferably in terms of what the patient does spontaneously or to the stimuli to which the he or she responds. Now, a prompt and lucid conversation implies that the person is alert and conscious. Now, memory loss for the accident commonly occurs with a conc- memory loss for items like your address, occupation, or year means intellectual impairment and early change consciousness. Now in progressive stupor leading to coma, lack of a verbal response to the question occurs sooner than does the lack of avoidance of there's an absence of painous or evoked. The general care, so you know, kind of just a sideline here before we go any farther, a lot of people they want to, they check the and oftentimes tertiary response. So again, we need its stress consciousness as soon as after the injury is able to document it oriented to their person, their place, time, year, occupation, whatever questions you happen to have and rely on if they're equal or unequal or things like that. Now in the general care, the vital sign, indicators of a change in the patient's status and you want them frequently and post them if we're in an battalion aid station or somewhere back further in a MASH hospital. Post them in a prominent place, they happen to have a blackboard on the wall. It's a good help so they can be charted back to the chart on what was it yesterday or 15 minutes ago or anything in a prominent place. Now a good airway, oxygenation and humidification or the utmost important is going to increase cerebral edema. The person is not exchanging air propsigenation. They're going to help hold any of that edema, swelling of the brain or the accumulation of fluid in there. Now, if the secretions pool, breathing becomes real labor, then you might consider putting in an endotracheal tube tracheostomy position. It means the person is resting on their side. The elevation of the head is superior to the person lying on their back. Now the patient should be given nothing by mouth until it's clear that no operation is going to be performed. It's infusion should be started and a Foley catheter introduced that the patient is not alert or if an operation is considered likely. You want to watch for the development of any water excesses because this is a common sequel to head injuries. You're going to monitor what you're putting in and what you're drawing out. If you notice that the person is retaining fluid, it's something to be concerned about, but bear in mind that it's a common event in any type of head injuries. Now if you're going to restrain the patient, you want to avoid in the spread eagle fashion. It's really dangerous. Now sometimes when people are starting to lose consciousness, they'll start grabbing at things whether it's the Foley catheter, your EKG leads, your IV, or the endotracheal tube or things like that. So, now, if the restraints are unavoidable, the patient should be positioned on either side and then you want to alter the sides every three hours with both legs and arms strapped to the same side of the bed. You can take very large bandages wrapped in a boxing glove fashion around the hands and that the force is going to help prevent any pulling at any of the tubings or dressings of your system. should be avoided because it's going to interfere with the evaluation of the consciousness. Now, the anti-convulsants are given to all patients who have had a cerebral contusion or any immunohymatoma. The trade name is Dilantin. It's usually given 100 milligrams. three times a day, that's TID, and it's going to be given IV after a loading dose of 500 milligrams. You want to avoid any intramuscular administration because it's going to have poor absorption. It's going to be 100 milligrams TID, IV, after you've given a loading dose of IV, up to where they need changes in the reaction to light. Now they may be caused by pressure on the third nerve or pontine damage. Pontine is kind of down towards the back. Dilated or unequal transitorial shift resulting from increased intracranial pressure. Dilated or unequal pupils. Good indication. This is going to be in addition to the person not being oriented to the person to place some things. And you're going to see that. And you're going to anticipate some of the things to tell you. Now, if they have constricted pupils, the signs of a brain damage, either primarily occlusion or secondary with increased immediate reduction of intracranial pressure is needed while the operating room is being prepared. If in mannitol, 400 milliliters of a 20% IV, then you're going to hyper... osmolar solutions are otherwise going to be avoided because what's going to happen is that's going to cause the, the mannitol is going to cause a brain shrinkage and then that's going to permit increased bleeding. So it's kind of a fine line between do we have to reduce the intracranial pressure now and immediately because it's really starting to turn on the brain and the brainstem. give the medication then it's going to cause the brain shrinkage and it's going to increase the breathing. One thing that we've avoided about angiography and echo scans and x-rays and things, I just want to touch on some of these things. I understand that a lot of people don't. Angiography is the most versatile diagnosis study evaluation of any craniosurremal injury, but it may not always be a valid hazard. and the gluizing signs may indicate the presence of an expanse of cerebral contusions in the dema and angiograms may help define the nature of the injury. And any echo, you know, you've seen women that they have their sonograms or angiograms of the baby and stuff, it's kind of a similar thing, you're using sound waves into the brain and stuff. Thesial applied noninvasively and it's a good test, an inpatient, and it can determination of the probable presence to lead to an exploration of consciousness may indicate the need for an exploration without either angiography or echoencephalon. Patient's condition is deteriorating rapidly. He's had to alleviate that pressure. Again, you know, we kind of touched, it's going to hurt you a lot more than it's going to hurt me, but usually by the time the person's condition deteriorated that much, they aren't feeling any pain. And we're back. Okay, we're back. Okay, we're going to jump right back into this, talking about some definitions and suggestions. Okay, the definition of a cerebral concussion, it means it's a temporary disruption in the cerebral activity resulting from injury. reflected in transient altered consciousness or other temporary loss of neural functions. Things like blindness, okay? Cerebral concussion means a temporary disruption in cerebral activity resulting from injury reflected in transient altered consciousness or other temporary loss of neural functions. A cerebral contusion means damage to a portion of the brain and is characterized by dulled mentation, pink cerebral spinal fluid, and a sensory or motor deficit. Frontal or temporal lobes may need to be resected if sufficient swelling occurs. The frontal lobe is right behind your forehead and the temporal lobes are above the ears. So again, the definition of cerebral contusion means a damage to a portion, brain, and is carorebrospinal fluid and a sensory or motor death. Now, in the next one, we're going to talk about linear fracture. In other words, there's been a fracture to the skull. Any linear fractures of the cranial vault usually are clearly visible in any x-rays of the skull. Now, if a linear fracture is present, the bacterial epithelia hematoma in an adult if the branch of the middle meningeal artery. Venous epitoma if it crosses the superior longitudinal sinus. And three epidural hematoma if there is a large separation of fractured bone. Basically you know you have big vessels that go between the left and the right hemisphere down the center areas. Again, you can sustain of the fracture. It could be an arterial epidural hematoma, a venous epidural hematoma, or a hematoma is a pocket of underneath the underneath this just kind of is a little bit of an anatomy and physiology. You've got the skin, you've got the bone, which is beside that you have three layers annoyed in the pia matter, and you have the brain on the inside of that and it depends on that bleeding is occurring as to where the relative seriousness of this is. The three layers, the meninges, they not only encase the brain, but they go down the spine and encase around the spinal cord too. I'm sure you've all heard of an epidural catheter. Women are going to go give birth and they'll place the epidural catheter in there to kind of numb from the waist down. This is your medication in there to adjust those parts. of the brain. So again, it depends on if it's one side or the other, whether it's the arachnoid, the dura matter, or the pia matter in there. The next definition is, is going to be a depressus. Now, it's an impaction of fragmented skull that may result in the penetration of the underlying dura and the brain. Now these fragments should be elevated depending on the location, the depth of the depression, and the overlying scalp lacerations. So a depressed skull, that would be as to where somebody has taken a blow to the head, if part of the bone, either partially or full, it's pushed it, definitely needs to be, the next definition is going to be a basal or fractures. Now they may be diagnosed with an x-ray by the leakage of any cerebrospinal from the nose or the ears, my bleeding from the ear in the absence of any obvious cutaneous cause. You know, you're going to make sure that if there's bleeding from the ear, make sure that it's not just that it's actually bleeding, that it's actually the bleeding is coming from out of the ear canal. So by any severe ecchymosis of the eyelids, that's another that you could have a basilar fracture and you're going to have a real big pock in, not actually bleeding out of the eyes first. And then into the back, underneath the ear bone down in the back that sticks, usually a good indication that the head there. So your antibiotics are indicated with any cerebrospinal fleek, cerebrospinal fluid leak, not with nasal or any ear bleeding alone. Now the ear canal should not be packed with anything since it might hinder the egress of any fluid or blood. You want blood coming out, you want to just kind of put a light dressing on there to absorb the blood, but you don't want to pack canal. That's going to do is then it's going to back up pressure. Now any inter cerebral hematoma, so any bleeding within their acute injury or a damaged secondary to any contusion. So you've got a contusion. accident, and their head gets the steering wheel or their head gets the windshield or something, then there could be the brain is actually bleeding on the inside. They may not have other than a bruise or a goose egg on their forehead or something, then it could be actually bleeding on the inside. It doesn't necessarily always have to mean that it's bleeding underneath that goose egg, you'll have the brain gets rattled inside there and it could rebound off of the frontal bone and then hit in the back on the occipital bone, your bleeding is. So don't be aware of the suspected side and the direction that the person was turned when the injury occurred. Any cerebral hematomas, these hematomas should be removed if they're being in the patient. Now any epidural, Hematomas may be arterial, venous, or origin. When a change in neural function indicates continued bleeding, craniotomy is going to be preferred to evacuate the cause of bleeding. The next definition is going to be a false aneurysm. Sometimes it's called a pseudo-aneurysm. It's a cavity with walls becoming organized with the piped tissue related in the source. Hematoma is a veritable amount of fresh blood, accompanied by suffusion of the anal usually in the severely contoured brain that is responsible for the patient's compromised clinical state. That again, usually it is for the patient's compromised and not the type of hematoma is fatal in up to 95% of the patients. So bear in mind everybody's, oh man, I ain't going to be drilling no holes in somebody's head. You know what? Out of 100 patients, 95 of them die anyway, so instead of sitting there and not doing nothing, you need to do at least something to try to alleviate the bleeding. 95% of the people die. You're telling everybody to go out here and start practicing brain surgery, that you're going to have to step up and you're going to have to get on with it. It's almost 100% guaranteed that the person is going to die. Any time that the operation goes on, it is usually about a 95% mortality rate. Now in your sub-acute sub-dural hematoma, that is going to imply that large amounts of recently clotted blood in the sub-arachnoid, either no contusion or the underlying brain or at the most, to it, cortical hemorrhage and small patches of sub-arachnoid blood. Basically, what I'm saying is you're going to have a little bit of the bleeding of the brain, but then it could be bleeding between those layers of the dura matter. I hear the music coming in.
