Yeah, I noticed that you know over the years they can't always come out and subvert a word outright, but they will usually slowly change the definitions of words until they lull us into a sleep. It was always called Armistice Day and that was a marking the cessation of hostilities at the end of World War I and then in the 1950s then they switched to the Veterans Day. It's now all of a sudden, it's like, oh, we have to thank all our veterans that are over here that have fought for our freedom and they're fighting for our freedom. Just for the life of me, I just can't understand where, as an example, what threat was Saddam Hussein in Iraq to our freedom? They talk about voting. That was one week ago. They sit here and they tell us that There, you know, these brave soldiers that protected our right to vote. What threat was Saddam to our casting votes on election day? But it's the die-bolt machines that are more of a threat than Saddam ever could imagine in his wildest dreams to our freedom and to our vote. Too true, Mike. Way too true. I wish it weren't that. Feeling better, just a little chilly winter has tried to set in here. I know how you feel, but I got a few reports. I told Mark this morning I'd call back in. He'd asked me to give a little bit of an update on the election here from what I saw here in my part in Indiana here. And I can tell you Chuck Baldwin was not on the ballot in Indiana. He took off of the air in between the commercials there. And on our touchscreen machines, Mike, there was no place you could scroll to when you went to write in your candidate. It just popped up a big keyboard on the screen and you had to punch in their name letter for letter. Wow. So if you misspelled it, guess what? It didn't count. Right. Now, the craziest thing that I saw was there was, you know, my girlfriend and I were sitting here watching the returns come in. There were several precincts that were showing zero coming in, no votes for nobody. And they were already saying Mitch Daniels was the winner. Now how could that be if all the different precincts hadn't been put in yet? How could they say Mitch Daniels was the winner? I don't understand that. Well, and I don't understand it either. Because here in Michigan, they declared Obama the winner. at about 7.15, 7.20 at night and the polls didn't close until 8. That's crazy. That's absolutely crazy. Who can believe that? You know, it's not over until all the counts are in, until it's counted. But the problem is it's not on the popular vote. People don't understand that. It's the electoral vote. That electoral college and this is this is something I've been talking to a couple of people about if you really want to make a difference Become a precinct delegate nine times out of ten Those posts set empty, you know happens to a post when it's set empty do you Because what happens is this they're appointed then that post is in appointed by someone Yeah, so who's the one that appoints them? Because it's not somebody from your area. They don't represent you at all or anybody in your precinct. Sneaky huh? Yeah, isn't that? Yeah, as a precinct delegate, you can go to the rally, the conventions. where the votes take place for each state, for the candidates as they're running the primary elections. Again, that counts more so than even your, goodness, the primary vote. Then you have the final vote and our popular vote. Yeah, it makes everybody feel good, go out there and vote. But that's not the one that goes by unless there is a tie in the electoral college. And that's where the decision is made. It's not made where you go down to your township and vote. So I think that's probably where our answer lies, is that that electoral college was done at whatever hour and that was it. because it was not a 50-50 split, it did not go to the popular vote, which is what our votes are counting as. Somebody had said that the electoral college doesn't vote until December 15th. The case that Mr. Berg is taking to the Supreme Court is supposed to be there December 1st. Now I received that from Dave in New York. He sent me a copy of the all the filings and stuff. I made copies of it and I took it to work and started spreading it around. Wow. Because everybody thinks I'm nuts. Even people that were a fella today, he's always been right there with me whenever I've mentioned something to him and his wife. They're an older couple, fairly older, and they were agreeing with me. And then today he said, I said something about, they're already laughing about they're going to take our guns. And he goes, oh, don't worry. The NRA will protect you. And I started laughing. And I said, the NRA isn't going to protect anything. They've been behind every gun law that we've got in the books. And he, all of a sudden, he goes, well, I'm going to say something that's going to make me very unpopular. And I said, dog, go ahead. I'm not worried about it. I make all kinds of statements around here that make me unpopular. And he goes, well, he started talking about the saying something about me and he wasn't very kind in his words. So I took it from there he was an NRA member. Well he needs to go back and do his homework. We're going to stick with us here. We'll be back. Everything is like McCordie was saying, you're in the chat room. They pretty much declared Obama the winner weeks before the election ever took place. Well, we know what's going to happen now. They've let the cat out of the bag. And I fully expect with the crisis the created crisis, being the door-to-door surge and seizure of firearms. And I believe this. I don't know if you heard me the first hour, but I see this happening pretty much the way they did here six years ago when they raided our home. They changed the director at midnight, and he implemented new regulation that we knew nothing about. At eight o'clock the next morning, they were knocking on our door. Come in to clean us out that simple and if you don't think it'll happen think again We're living proof. It can and will happen They'll start it and they will declare martial law I wouldn't be the least bit surprised if they didn't change our monetary system and went over to the blueback dollars or the emeros You know, maybe, you know, again, because they are using this Congressional martial law issue to press things through, and they're already promising this. Not saying that specifically, but there will be no discussion, there will be no debate, we will just vote on it. Well, you know what, I'm to the point, it's like this, and something I want everybody to do, and I mean every one of you. You need to call your congressmen, your senators, and tell them simply this. We'll get to our call in just a second Ed. Tell them this. If a bill cannot pass on its own, let it die. If it cannot pass on its own, let it die. Then it is not meant to be. Don't be tacking it on to another bill to pass as law. That's ridiculous. Oh, but that wouldn't be any fun, Nancy. They'd have all kinds of tags left over. I don't care. You know, it's so simple. Anyway, I just wanted to let everyone know that we're having a militia training exercise in Texas. Militia participation in Texas has declined since the militias throughout the United States back down the gun-hating Clendenistas in the 1990s. And now the Muslim Abominite regime is coming into power in the district of criminals, and there are even more anti-gun than the Clint Nieces were. They want to enact a new assault rifle ban by executive order that will never expire. So I'm asking people in Texas to please help us bring back the Texas militia to full strength. It's very important you get out there. Guys, if your wives or lady friends have not been out with you, they need to be accustomed with rifles. I would highly suggest that it not be you. I have seen more arguments between loved ones over training with a firearm. It's not a good thing. Get somebody else to do it. Somebody you trust. Trust me. It'll make your relationship go a lot further. I know it sounds funny, but been there, done that, not a good place to be. And it's, yeah, we've been down that road. So that might be a good thing for you to think about. How well do you really get along? Trust me, if you're out in a training and you're bossing your partner around, Not fun. It can get real ugly fast. Oh goodness. Anyway, Michael, I'm going to turn some time over here. You probably have some medical things that you want to get into. Am I right? Oh, absolutely. I'm always ready for that. Some we haven't really touched on lately, the medical aspect of the things. This next block, it should go relatively quickly here. It's going to concern facial injuries. This is still a continuation from resuscitation from shock and trauma. This is the next block of instruction before we were talking basically about assessing the patient and your priorities and pumping the patient back up with IV fluids and assessing how well affectation or doing. Now, this block of instruction is going to cover facial injuries. Now, patients with the facial trauma may have associated mortal injuries, but just facial injuries in themselves rarely cause death. The most common serious injuries associated with facial trauma involve the brain, the cervical spine, and the eyes. Despite any frequently published statements to the contrary, serious facial injuries an indication of an emergency tracheostomy. Tracheostomy have associated injuries with the head and the neck and the chest. It's a real common cause of your facial trauma or automobile accident. Now the classification of the injuries. Now to avoid overlooking any forms of injury, underlying type of injury, we're going to have to develop a scheme of organization of common facial injuries And so we're going to try to put this in so we can cloud. The simplest scheme is going to divide both areas in the facial bone of injury. Now, number one, it's going to be your soft tissue facial injuries. And the type of injuries are going to be number one, it's going to be contusions and abrasions with or without a hematoma. Two would be an accidental tattoo of numerous small foreign particles embedded in the dermis or even retained larger foreign bodies. A third category is going to be puncture wounds. The next one is going to be lacerations, whether it's a simple or a beveled or a tearing or a bursting type of laceration. And the fifth one would be an emulsion injury. And that's either with a complete loss of tissue or as a flap type of injury. Now, an emulsion, it's not a straight laceration like a cut or a knife cut or a thing. to where it's kind of sometimes it'll be a flat skin is a flip back over consideration is going to be the location of the injury whether it's in the forehead the eyelids the ears the nose the cheek on the eyes we can have a so we can have an oak as a next one or is going to be the location of the fracture and again now as you're evaluating these and treating the patient the the soft tissue injuries the facial bone fractures and the location stations of fractures, radio to the battalion aid station or things in the field back to our battalion aid station so then we can all be talking on the same sheet of music. This next category is going to be the location of where the fractures are going to be. The first one is going to be the upper third of the face where it's the supraorbital ridge. Supera means above and the orbitals are where your eyes are, so it would be up where your eyebrows are. That would be the super orbital bridge that runs across there. The next classification or the location of the fracture would be the middle third of the face, then that would include the nasal bones and the septum, the maxilla. zygoma and zygomatic arch, those are your cheek bones and then your transverse maxilla. I hear the music coming in and you can go ahead and lay your pencils down and we'll be back within a couple of three minutes. We'll continue on with this. We'll talk about the priorities of treatment, the observation and the palpation to try to get a pretty good handle on these types of injuries. to block construction isn't going to be super long. It seems this year's drought has caused significant problems with the milk production. The lack of rain has affected the muck the dairy herds are producing and causing the mucking machines to become plugged up. Mike, you were continuing on with a portion on facial wounds. Yes, we were talking about the location of the fractures. Now the third category is going to be the lower third of the face, the jaw bone, the mandible, and all the parts, the alveolar process, the the body, the angle, and all these big fancy names. Basically, it's the lower jaw bone. Now, the next category is going to be the treatment, but just as a quick review, what we've covered now is the classification of the facial injuries. We talked about the soft tissue facial injuries, the typo. the injury, the contusions or abrasions, the puncture wounds, the lacerations, and the evulsions. The next one was the location of the injury, whether it's the forehead, the eyelids, the ears, the nose, the cheek, the chin, the lips. The next one would be the facial bone fractures. What type of fracture? Is it as closed? Is it open and green stick or displacement or undisplaced? The next one is the location of it. the upper third of the face, the middle or the lower part. Now the next big category here is going to be the priority of the treatment. Now although your facial injuries, they can be very expensive, the patient first must be evaluated completely using the priorities that always apply in trauma and that are outlined in the previous part that we had here about the resuscitation from shock and trauma. Now, expensive facial injuries seldom cause great pain. When the pain seems to be severe, you're going to always think that there are associated injuries. You want to suspect that there could be something. So bear in mind, faces and fingers and things, they have a tendency to bleed a lot, but in general, there's not a whole lot of pain associated with them. So if a person appears that they're in great pain, then I want you to start thinking that there could be some other thing that's associated with the injury that you need to suspect. Most of your facial hemorrhage can be controlled by direct pressure or to tie in off the legation of the bleeding vessels. Now, one real danger of facial hemorrhage is the obstruction to the upper airway and the volume of the hemorrhage from facial injuries alone is seldom sufficient to cause any shock or to require any emergency blood transfusion, but associated cerebral, cervical, or ocular injuries are most apt to be the cause of the shock. So, apparently, the surgeries are stuck and somebody's got a bloody face into that mindset that this is going to cause a person to go in shock. but they could be if they have an automobile accident, they have a facial laceration, we really want you to start thinking that they could have a cerebral or cervical one on the inside that you really can't see. Now the diagnosis of your facial injuries is what we're going to establish that by observation and palpation or also ECLI, the x-ray part off because I know most people don't have access to Now in addition to the obvious soft tissue injuries, asymmetry of the face caused by the underlying fragment can be detected early before the masking effects of the soft welling occurs. So it's very important. Initial treatment of the patient that we check the person out asymmetrically, you're going to touch each cheekbone at the same time. Each side of the jaw at the same time, the mandible and the maxilla and the forehead at the same time. You're not going to use one hand and go around in a circle feeling these bones. You want to start and work your way around on the facial bones, up on the forehead and around the nose and around the eyes equally on each side. What you feel on the left should be present with what's on the right. You need to do this before any swelling occurs. After a while, say for instance, you treat them to stop the bleeding and then you evacuate them back to a battalion aid station that may take an hour or so. Then there's a lot of swelling that's occurred and then they may miss. an underlying fracture because of the overlying swelling that has occurred on there. So it's really important to check this out to make sure that none of these underlying injuries are being masked by the soft tissue swelling. Now the eyelids always should be open to look for any eye damage and the mouth should be open to observe for any inter-oral injuries. Make sure there is no bleeding, no teeth are missing or loose. You can always evaluate the teeth. For instance, if you have a Styrofoam cup handy, put that between somebody's mouth and ask them to bite down on that cup. You can evaluate the upper and the lower jaw that way. palpation, the observation of the facial symmetry is reinforced by the palpation of the bony prominence, your zygomatic arch, the bridge of the nose, the bridge over top of the eyes, the mandible and everything. Again, we want to do this symmetrically. Now, the landmarks may be masked by hematoma or edema and comparing the height of the different prominences on the face are going to be very informative in the diagnosis of any depressed fractures of the bone. Tenderness usually could be elicited at the site of any fracture, but seldom is the discomfort really extreme. Again, it's really important to just check it out all the way around and make sure the next step is going to be the triage and the treatment of the facial injuries. Now, once any life-threatening problems have been resolved, then we're going to tend to the soft tissue injuries that we can repair under any local anesthesia and they need to be treated first. Now, any complex spacer injuries with tissue loss and extensive fracture, when it's got to be posted. Now if necessary, you can do the soft tissue injuries, can the weight without any repair for optimizing any final results? Problems have been properly cleaned, antibiotics are advisable. If there's going to be any delay and any of these soft are going to be repaired, basically what we can do instead of trying to go around and suture all the way around a big avulsion, then what we can do is just suture it along the sides to kind of just keep that tissue and then that tissue going to, you know, out and then it's going to die and then it's going to have to be trimmed away and then leave a big ugly, you have to get onto them right away and you might be able to evacuate somebody back to a moment where they can be treated a little bit better. Now any reduction in fixation of any facial bone fracture need to be carried out as an emergency type of but weeks after the injury origin, the healing begins with a fibrous union which usually can be overcome by any manipulation. Alignment children is accelerated and reduction of the attempted usually about seven type of carol injury should be oriented toward anatomically repositioning of the soft and if this features the symmetry, the dental, it will be usually restored. A little bit about outpatient with acute extensible injuries, candidates for general anesthesia. Sometimes they'll be arriving at the emergency room. Their stomachs are going to be filled with food. And then given a reasonably co-option, local anesthesia is definitely the superior for the management of the surgeons work since an endotracheal tube, and that's going to be required for general anesthesia. usually interfere with the results because access to the patient's failure to the age and the condition of the patient will make local anesthesia. Repairs of the deep structures of the face and open reduction of what we do is a weak solution of epi- did in the face and that's going to have the hemostasis and then the shortening the operating time. Remember the epinephrine is a so we want to dilute that we're not going to put in full strepfering in there like if it's light of cane with epinephrine. So we're going to dilute that down and because boogers are really vascular they have a tendency to bleed a lot so if we infiltrate that into the area that we're operating on and then when we're going to suture it shut then there's not going to be a lot of associated bleeding with the repair of any softening. In the operating room you're going to need at least the doctor or there's going to be the sewing. You're going to need a scrub nurse and maybe somebody else a circulating type of person. and they're going to be needed to assist in the repair of the facial injuries, you know, and because they aren't attempting any soft tissue repair unassisted in an emergency room or a battalion aid station type of deal, it's only going to invite frustration and it's going to give you second tissue loss. Shoe losses can await definitive repair until the patient, sometimes we can repair this with adjacent tooth without any distortion of the features, it's going to be If you have to go to skin grafts, you'd want to try to use a press to match the finishes up that block of instructions and the repairs of those injuries. So, we really didn't ton too much again with a whole lot of the, you know, this is something that you may see out in the field, but to actually start to treating on that out in the field is very seldom seen. But I'll maybe touch a little bit of this in the next bowl injuries, we'll talk about. and what to do and when not to do about, well, if the person's consciousness or if the pupil on one side is fixed and the other one's dilated, we suspect that going on can add a little bit more heavier in there. So I've kind of skipped over the x-ray examination, again, because most people don't happen to have an x-ray or the ability to read x-ray. Of injuries, of patients, of the fractures, the priority, the treatment, the observation and palpation. Nancy, do you have anything to add on that? Oh goodness, you're so thorough. No, honestly, now as long as people remember, head injuries are the ones that bleed the most. I remember Ed terrifying his teachers at school. He bumped his head, got a little cut above the eye and just bled all over himself, soaked his shirt, soaked his jacket. The teacher about passed out. But getting him you know into the nurses office and find out it's a little tiny cut above his eye It was you know very very little Not even a half-inch long, but blood like crazy I know Ed remembers that all too well Terrifying the teachers there, but he was good at that. Well when they found me I was unconscious under the monkey bars. Yeah whacked his head good. And apparently I've been bleeding long enough my the whole front of my shirt and my face was covered with blood the shirt was soaked red. Yeah it was yeah he had them terrified. They were oh my god you gotta come you gotta come now. So yeah ran down and the nurse had it and cleaned it up and and? He needs a clean shirt okay. But other than that, you know, it's amazing if you've ever dealt with them. It's just the littlest thing. And the thing about head wounds too, the deeper the cut, yeah, this goes pretty much for most wounds. The deeper the cut, the less it bleeds. So if you get a head wound where you've got a deep cut, you're not going to see as much blood. And those are ones to be more concerned about. So, you know, unless you cut a bleeder vein that is, you know, spurting. That's something to think about. If you see a lot of blood, that's what your body does, it uses the blood to clean the wound. It's a natural effect. So don't be totally freaked out, especially with head wounds, because they're going to bleed like crazy. Yep, faces and fingers. Yep. They're all good things. So it's just your body doing its job. It's just fun. Go ahead. To not become so fixated on the laceration that what we have to think of, of what's going on inside was the blow to the head hard enough as to whether there could be more bleeding inside the cranium or it could be a cervical injury and don't become so fixated on stopping the bleeding on the forehead. Say for instance there's a motor vehicle accident and somebody's head struck the steering wheel that will whip a bandage on that and we'll have a tendency to overlook the hidden injuries. It can be much, much worse on that. A lot of mothers freak out when they see a knot on their child's head and actually when you see that big bump occur, that goose egg if you will, that's actually a good sign. Because if it's coming out, it's not causing pressure on the brain. You know, if it happens almost instantaneously. The ones you have to worry about this day flat, and then you have bruising on the brain itself, and the continuation of, oh goodness, bleeding inside. And that becoming a problem. I lost a very very dear friend that at the base of her skull started bleeding internally in the brain and within 24 hours was gone. It affected the way that affected her was was very interesting because she she started not making sense, normally very lucid individual. and talking nonsense and after just a slight bump to the head. It wasn't a big fall or anything. But the family took her to the hospital and 24 hours later, with the bleeding in the brain and they couldn't get it stopped, she was gone. Mind you, she was... Was that a called a pontine bleed? I don't recall. I don't recall. It all happened so fast. It was... It was quick and of course she was an older lady. She was about 75, between 75 and 80. But as you get older you don't heal as well either. So it was fast. It was real fast. And there was nothing anybody could do for her, unfortunately. So, you know, somebody to keep an eye on, you know, that if it doesn't, if you don't have that bump, it's almost better to see that big goose egg than not to have one and wind up with concussion or bleeding in the brain. Head wounds are nothing really to mess with you know if you know what you're doing It's one thing, but it's it's a tricky situation because that's your brain folks That's what controls your body your organs your involuntary as well as voluntary body functions Really important to take care of those injuries ASAP anything else Mike No, that's before, you know, I didn't want to launch into the next about the head and cervical injuries. That's kind of a bigger part. I just kind of wanted to touch on this about the location, how we can categorize these, and just the treatment and the triage and any ideas on how to protect it. And one of the big things is, like I say, A lot of these injuries, their treatment, their full treatment can be postponed for a while as long as they get, the wound has been cleaned, cleansed, wait till the patient's been evacuated and then we can get into trying to do something out in the field where you got dirt and people walking around.
