For those of you who are participating, Chuck Baldwin for president, by the way, Chuck Baldwin has been recommended by Ron Paul. And Ron Paul, well, he should have stayed in the race, guys. I know that. But Chuck Baldwin is registered with all 50 states. Hey, let's see what happens if we put the boat in and wherever it goes with a die-bolt machine, maybe we'll actually see one or two acknowledged out of however many millions of votes are cast. You never know. Notice I'm being facetious but I'm not because we all know what's going on with the lying machines. On that note, most important is we would like to keep track. I know a lot of areas where we have independence. At least pay attention. Try to see if you can get a group of people to go in just like what we did with Ron Paul. Vote in a block and see if the machines go offline. Remember what happened right here in Michigan we had this happen in several precincts. They were in first thing, the guys went in to vote, they cast their vote and after the first or two or three machines registered for Ron Paul, all of them went offline. Some female came out with a yellow pad and said, oh write down who you want to vote for on this yellow pad here and I'll take care of it later. Wait a minute, what about the machines here, or for that matter, how about if we just break out the paper ballot here, and oh wait, we can't do that, they made sure that process was buried, or burned, and for obvious reasons, so that that person with the yellow pad is supposed to be trusted to do the right thing. You were supposed to be able to walk in, use whatever piece of equipment it is, and it was supposed to work. Not that that's a guarantee anyway. But the ballots would be the best choice, still are the best choice, and once everything is said and done, it is the only form of voting that will be allowed in the future. The machines are going to the scrap yard. There will be no more machine counting. And we don't need to know the next night or even that night who won. If it takes two, three, or four days to accurately count, that's fine. And if anybody tries to interfere or alter the numbers, that will be a high crime. That has to be the agenda. We need to plant that as a seed and that is part of our future for America to ensure sovereignty. Okay? Now Mike, you were bringing up something before we started the program. Some interesting little data here on the candidates coming in right now. And what is it you caught from that end? Okay, very good. I guess here's the disclaimer. I'm not a fan of Randy Rhodes on Air America. I monitor both the right wing and the left wing programs. If anybody else does, I'm sure you've heard this name, Khaledi, a Palestinian. I've been kicked around quite a bit. The right wing want to call him a terrorist and this and that and who and there. I guess a few years ago Barack Obama attended a dinner party for him and stood up at the end and made a toast. Somebody was taping the event. They turned the tape over to the LA Times. They gave it to them anonymously and told them that they couldn't release the tape. Not that they couldn't write about the tape, but that the tape would not be released. Well, today I'd switched over from the right wing over to the left wing and it was Randy Rhodes was talking about the Khalidi gentleman. It seems to be now, I haven't quite got all the dates, but I'll just kind of throw them out there as reference. Khalidi started this organization called the Center for Palestine Studies. Then along comes another organization called IRI, it's the International Republican Institute. Now I have written down here, it says started in 1993 and I'm not sure if that's the Center for Palestine Studies or the IRI somewhere. But somewhere along the lines then in about 1996, John McCain became the chairman of the International Republican Institute, kind of an affront parent organization for this studies. John McCain was on Larry King Live last night and this about Khaledi came up. John McCain made some comments because I guess Larry King had asked him, well you know the Palestinian is a terrorist, you know. John McCain I guess said something along the lines of, we really don't know about this guy. Like they say, John McCain was the chairman of the board and still is of the International Republican Institute. Somewhere back in there around 1996, John McCain gave the IRI $448,000, almost a half a million bucks, Khaledi and his Center for Palestinian Studies. So now you would think that if I was going to give somebody a half a million bucks, I'd certainly know who they were. But it was just odd that the way that they played it, Larry King asked a question and John McCain says he didn't know who this Khalidi was. The other deal is, this tape is being held by the LA Times in confidence. They wrote an article about the possession of the tape back in April and saying that they had the tape. So when the right wing comes on and says that they're trying to hide this thing, you have to bear in mind that it was the LA Times that wrote the article that they were in possession of the tape. So if they were really truly quote unquote trying to hide the tape, they wouldn't have written an article about it in the first place. Then when Randy Rhodes wrapped it up, she did say that she had found all this information in Harper's Magazine and then the New Yorker Magazine. It was on the first hour of today's program and today's date is the 30th of October, 2008. I know she's got a podcast or archives or something. Sometime after the half an hour, she can look up Randy Rhodes or Air America. It was kind of an interesting naming. It was just odd that we hear all this stuff about Khalidi and the speculation. I guess there's also something that he had in this Center for Palestinian Studies, I've heard the right-wingers trying to call for Palestine Studies as a nonviolent. They want to educate the people to break this. He was actually opposed to Yasser Arafat and the PLO and the Palestinians like that. When we hear this stuff about the Halidi and the Terri, we have to ask ourselves, why is this question about John McCain giving almost a half a million bucks to this organization? Why ain't nobody talking about that? Mark? All they do is say a suspected drunk driver and that's pretty much it. They don't report it on their NBC affiliate as an illegal alien and they don't report it in the newspaper as it is. Now they're talking about running some big sob story about Oh, well all the violence against the illegal aliens, but they're afraid to report the information to the police. And then a little bit further in the news report they say, but we don't have any evidence or any statistics backing this up. So, you know, if somebody's sitting there and wants to look at the left hand, look at the left hand, and, you know, they don't even have enough backbone to... talk about the illegal alien that was drunk driving. Oh speaking of that, I was listening to the right wing last night, the J.D. Heyworth's program on KFYI, that's KFYI.com. I guess there's somebody from the Hispanic Bar Association. I called in and I was inquiring if the illegal alien belonged to the Hispanic Bar Association right before he had ran over the Phoenix police officer. They were talking about it and now they've got the letters from the Hispanic Bar Association that have been sent to the President and the President-elect. of this organization and it's made it all the way to the superior court here in Arizona that terms like illegal alien won't be used. It's a trial balloon right now. But you know how they do. They float it out here until everybody gets the message and then they find you in contempt of court. But pretty much from now on the term illegal alien and other terms along those lines will not be used in court. And they have the letters, the official letters from the Hispanic Bar Association and the Superior Court and the whole nine yards. They're all posted there on KFYI. I don't have a computer. I'd find them and I'd print them out. Well, here's the interesting thing is they push this through the bar and the bar is yapping their feces up, or what? How do they claim it? Did this come from the Hispanic Bar Association? Read that the forward advance party, the avant-garde for the rest of the scumbags are going to be trying to create the as-lan scenario? Or did this come from traders in the bar? Oh, wait a minute. There are traders. They're in the bar. So they are traders anyway. But for them to engineer this political correctness from that direction, where did that come from? Which direction? And that I don't know, he did throw out the woman's name, her new name was Ruth McGregor. Now that doesn't sound like a Hispanic name. I suspect that she's something that has to do, I think she's a judge in the Superior Court. And I don't know if she's the one that's implementing that or she's the liaison between the Hispanic Bar Association. I was outside doing work and I didn't have time to stop and take notes of about everything he had said, the letters and everything were posted on their website, but as to all the particulars. It's just an example. It seems to me there was a year or so ago there was something, and it wasn't here in Arizona either, but there was some rape case, but they had forbidden the victim from using the word rape during the trial. It's not just illegal aliens, it's quite a few other... We can't say these terms that might influence the jury with these words that they've been polarized in a negative way to respond to. We can't say rape, we can't say illegal aliens, we can't say this, you can't say that. They say they are not trying to take away your First Amendment. members of royalty brought in to force admiralty law upon us and this is just one of the many little manipulations that they're doing for business purposes. Remember that they're in business. There is not a concern for this country. In fact, they have just a reverse agenda. Their objective is to be the fifth column that will help to destroy the sovereignty of this nation. That is their agenda. Period. So we need to stick them right on down the road too. We don't, you know, just as tempered with mercy I'll remind everybody of this, although again many of these characters are flat out traitors. Most of them, deportation. That'll settle the whole problem. We get rid of that problem. We're not going to have to worry about them fighting or coming around a backstabby down the road. We're not interested in any mass executions. Wrong, wrong, wrong, wrong, wrong. We're not going to see that. There will be trials for specific acts of treason and they will be dealt with accordingly. But most of them, we're going to deport them. Now they're not going to get to be deported this time to where they want to go. We have the wherewithal to take them where we want to deport them. And I think that's going to be the most critical thing. So for everybody listening out there, when the time comes, we're going to be dumping them on people that probably aren't going to want them. That's OK. That way they'll be treated properly at the other end, and our hands will be washed of the situation. See how that works? So I want everybody to keep thinking that way, because that's where we're going with one of the several solutions to deal with conflict after this war is over. Deportation is a key word there. Justice tempered with mercy. Justice tempered with mercy. Remember that term? I've used it many times and I will continue to remind you of it. That this way they're not part of our problem anymore and we wash our will. We'll be able to get rid of them. You know, get them down the road. If their ideas work, they're going to suck up to some socialist parasite over wherever we drop them and apparently the world will be idyllic wherever they go. But they won't be dragging us down anymore. And that's really one of the things we need to consider and look at across the board people. Think deeply on this subject. See, we don't want the French Revolution. We don't need the craziness of what happened there was intentional. It was not an accident. It was planned by the money manipulators. And I think number one, I think they were frustrated, Mike, because they couldn't get away with it here. It didn't work the way they planned here. Everything that they know, a lot of that became a real feces pie for them when they thought they were going to manipulate the situation and they'd have the same mayhem and confusion. It didn't take place. People were of like mind. Now this time around with the nonsense they're doing with Osama bin Ding Dong and the chipmunks or with McLean, they do feel that they're going to have confusion. We're not confused. We were discussing over the During the break that it's kind of tedious to go over this chart, so try to give you some references here Just bear in mind that the chart it shows the the different accesses of the blood and the donor's type of blood and that the O negative blood can be given to any patient and that any patient with AB positive blood can receive blood from anyone. And of course, you know, now there's further subdivisions that make the whole blood transfusion not quite as simple as the chart has showed, but at least it's a starting point. Basically, and this is another thing that we've covered numerous times, is the simplest to do is have your group go down and prior to any disasters or expeditions or everything, to go down and have everybody donate blood to a donor card on there. For economic reasons, usually the ABO and the RH has been done with a liquid anti-A and an anti-B and an anti-D sera and the anti-D test, whether if it's an RH positive or an RH negative. And they'll put the blood drops on a slide. And then if they clump together, then you can tell that. drive the anti-A and the anti-D and the anti-D Sierra and put them full test on a pre-made car. We acquire these cars and usually get them for somewhere between six and ten dollars and they're becoming more and more popular because of found out that a lot of times their diet and they usually have a shelf life of about three years but the drawback is that they have to be strawed in the refrigerator. to consider that again the best way to do it. Just go down and have everybody in your group donate some blood and then you'll find out what everybody's blood type is. A week or so ago we were talking about the choice of olemia covered over the crystal. The initial volume replacement with nocillium or cated ringers, it's going to help put a fluid back in the veins, it's going to help pump the person up. And now there's always been the big discussion, the theoretical advantage is to a buffer. the It's desirable to maintain a hematocrit of above 30's values. The oxygen carrying capacity can protect against further hypoxia. See what happens is if the person has lost a lot of blood and we just put a bunch of lactated ringers or normal saline in there, they still don't have the hemoglobin in the hematocrit to be able to carry the oxygen. So that can be a contributing factor to low oxygen content. then the dilution of the hemoglobin is going to be a limiting factor in the volume of the crystalloid solutions that you're going to be given. Now, what we have to really avoid is that there is absolutely no place for your soltoloid solute W in primary resuscitation. What's going to happen is if these solutions are given in acute post The water is going to be retained because of the action of your anti-diuretic hormone and then you're going to get into severe cosmogality as a result. So the person is going to actually start retaining water and then you're going to be fighting against yourself because of the endocrine response and the ACTH and the ADH. And then so this is going on as the body, the hormones are going to say, hey, you need to stop making urine. it's going to release the antidiuretic hormone, the ADH, and then when you start with IW, and then you're going to be in there, so it's really going to be going against yourself. And a couple other things just as a quick of some of the stuff that's human. Now, human, the serum albumin in various concentrations is usually readily available, and it's free of a risk of hepatitis. It's an effective plasma expander and it also has a lot of other advantages. It's easily metabolized and it's spare muscular proteins from cannibalized to the bloodstream here. The one problem is though, if you give too much, then you could get an accumulation of the albumin in the pulmonary interstitial space and then that can lead to adult respite, so you've got to be administering large volumes of albumin. Other other times are referred to as plasmonate. They can be given without cross. You should be especially considered when plasma loss is anticipated. If things like burns or peritonitis or pancreasitis and then the use of fresh regulation factor abnormalities and stuff. And I understand we're covering some of these things. We may not have access talking about that. Now the dextran, plentiful dextran and low molecular weights. and they also decrease the blood viscosity in the micro circulatory flow or might interfere with platelet function. And the use of dextran to exploration of soft tissue injuries is ill advised. So if you have somebody that's lost a lot of blood for gunshot wounds, you might really want to hold on the dextran especially if you're anticipating evacuating them back to an area where they're going to have surgical procedure for the injury because it's going to interfere with the platelets, the clots. So if you get the dextran in there and then you go and take the dressing off to do an exploratory surgery or to suture the wound shut, then it's going to just start bleeding the whole lot through the soft tissue and then it's just going to be a real big mess. Although it's often effective in preventing renal failure, low molecular weight dextran be responsible for renal failure due to a tubular burn, very hyperosmolar urine, four-volume of the urine. No more than one liter of dextran solution should be given in any one day. Now for, to touch on about blood transfusion, now major losses of whole blood can be replaced with whole blood with a plasma substitute like albumin or plasminate or fresh flows and plaque. Now it's generally possible to provide an adequate volume expansion initially with your crystal can be accomplished and the use of matched blood minimizes any reaction due to blood grouping and compatibilities or anything like that. Banked blood becomes progressively acidic and hyperkalemic and there's a rapid loss of many of the coagulating factors. In addition to a high incidence of transfusion reactions with massive transfusions, greater than 20 units. It also carries the potential of causing the development of what we'll call a wet lung due to a pulmonary entrapment of fiber and other particles that are in the blood. So you're going to have to make sure that you use a microfilter to run the blood through. And so that kind of wraps that up about some of the things that you might want to consider having to pump somebody back up to an acceptable level. I hear the music coming in. Yeah, we're always talking off the air. We got a few minutes to go back and forth. Mike, you're going to pick up and again, we're going to touch on a few things, especially with IV packs. And I want to qualify something. Just because we don't have it at the moment doesn't mean you aren't going to get it. In fact, I run into stuff all the time. I just tell Mike, you know, we got like boxes, cases of saline for intravenous feed, but in glass. Some of them quite large as a matter of fact. It's interesting, it's purely a matter of, you know, if you're in an area where there's a lot of hospitals, there's a lot of unrestricted medical supplies that either date or whatever that are going to be perfectly fine for field operations because you're not going to ask, hey, is that outdated by three months or a year or two? Well, I'll tell you what, let's put it this way. It's either I use this or nothing. Oh, okay. Now here's the other reason I don't have a problem with using quote-unquote outdated like medical tubing and IV and butterflies. How many of you have seen all these movies where they show these guys having to improvise with radiator hosing, rubber black hose from pneumatic systems and all this other BS to make an IV or whatever? Now why should we do that if we are so resource rich with the technologies that we have at hand? Now granted I want to know how to do that if I had to, but I shouldn't have to. You see what I mean? We have the resources. Understand how to use them. Mike, take it away because we've got enough time to get that in here, please. Okay, very good. Just as a review, you have somebody that's volume depleted and you need to pump them up. Your primary would be your lactated ringers, usually referred to as LR or RL. Ringers, lactate or lactated ringers, they're both the same thing. They're just said a different way. If you're going to consider giving someone a blood transfusion, the only solution that is acceptable is normal saline. If you give blood in a ringer's lactate or in D5W, the blood is going to clot in the tubing and you're going to have a real problem. So, initially, if you're going to have a choice, we're going to go with LR right away. And again, you're going to have to estimate on how much blood the person has lost. You can need to be prepared to give them up to four times the amount of blood that's been lost. Usually, I think we covered that the other day, and it was somewhere about a little over a liter. If you can stop somebody from bleeding before they've bled out a liter, there's usually enough volume that's still in their body, so you're not going to have to really consider trying to think about having whole blood transfusion or albumin or plasminate or anything like that. So your primary concern is going to be stopping the bleed. Well, first off, your first concern is going to, initially you're going to make sure that they have an open airway. Then we're going to stop the bleeding. And then we're going to, depending on the amount of bleeding, we're going to start an IV. I would go ahead and start an IV at first because especially on adult males, they have a tendency to be able to maintain their blood pressure and then all of a sudden when they reach a critical point, all of a sudden it's like a wave that comes over them and all of a sudden everything flattens out. You're going to, as we pointed out in the ABCs before stuff, you're going to look, you're going to check the area in all ways. You're going to look in 360 degrees, you're going to smell in 360 degrees. You're not going to do anything stupid and run out there and become a casualty yourself. After we've checked the area, then you're going to get to the patient and you're going to open the airway, and then if they're not breathing, If there is not exchanging air, then you are going to initiate the breathing. Then you are going to stop the bleeding. Then your primary solution is your lactated ringers. Then if you are going to anticipate going for a blood transfusion, you are going to go to normal saline also depending on the amount of blood that the person has lost. You might even want to consider starting multiple IVs. Now say for instance they've been injured in one arm and say for instance their hand is blown off. It's not going to do any good. to start an IV in that side because a lot of your solution is going to be going down and out through where the hand used to be. So you're going to go to the other side and start an IV in that side. And then you might also want to even consider, maybe even if you have to, starting one in maybe in the neck in the external jugular vein or underneath the clavicle in your subclavian veins. or or in the leg the only drawback is if you if you think about going subclavian it's so close to the lungs there's always the possibility of puncturing along that way uh... talking about medical supplies alphie uh... sent me an email and uh... the found this one that a medical uh... medical and veterinary site it's called shot m e d two one eight hundred seven 521-800-544-751 Shop Med Vet They have a load of stuff, anything from pharmaceuticals, instruments, syringes and needles, suture and skin care, surgical and exam room supplies, IV administration, vials and containers, cotton products, gauze, lap sponges, wound care tape, bandage dressing, scrubs and apparel, gloves, anesthesia products, diagnostic equipment, laboratory equipment, x-ray products, orthopedic products. miscellaneous reference, pet grooming, restraints, dental products, sterilization, disinfectants, and cleaners. You name it. They have quite a bit of stuff in a wide variety. If you're on the low end, you can buy, you know, hemostats or needle holders for a couple of bucks. Or if you have a little bit more, they have them up here in a couple hundred dollars for some super high quality. needle holders and things like that. So it's a wide range of a variety. Shopmedvet.com some of the things that you're going to consider. Instead of everybody having liter bags of solution, it might be a good idea to go to the 500cc so everybody can have a bag of LR and a bag of normal saline. That would give you more of a choice of what to have. and then you wouldn't be restricted and you're also carrying the same amount of stuff in the same space because a 500 cc and a 500 cc equals one liter so you can have a better selection of the LR or the normal saline and you're not going to be just stuck carrying one bag or say for instance what happens if your bag of LR gets punctured and then you've lost all your fluid It is better to have two bags and hopefully none will get punctured. But just in case you fall and it bursts or something like that, it is better to divide yourself up. I would have a tubing. I would have usually the butterflies or the gel coat for the use for starting an IV, I'd have make sure the patient's allergies, I'd have a way to prep the site, I'd have a choice of either betadine or alcohol. If the person is allergic to shellfish or betadine or things like that, then if all you've got is just a betadine, then you're stuck. or you can also just use water, just plain water. Open up a fresh brand new bottle of some bottled water and flush the area with plain water. It's better than nothing. The only problem is if you're using betadine, the tape is not going to stick to that. So you're going to need to have the betadine and the alcohol. And after you've prepped the area where you're going to do the venipuncture, and then you're going to strip off most of the the betadine with your alcohol, whether it's a prep pad or pouring some on a cotton ball or something like that. And then go ahead and you're going to need a tourniquet. to help to extend the vein. You don't always have to. You can have somebody else just kind of put their hand around the person's arm and that will help to extend it. Or depending on the temperature, you might not even need that. If you just lower the person's arm below their heart level, that will usually cause the blood vessels to distend a little bit. And then I'd have at least two needles of what you're going to use in case the first one doesn't go and you're going to have to pick another site so you need to have two bags of solution, one ALR, one normal saline. two of your IV needles, one tubing, I'd have a pair of gloves for the person starting the IV, I have something to use as a tourniquet, you can always use a spare exam glove to tie that around there, and then I'd also have some tape in there that you can tape the the tubing down firmly after you've started the IV and get that going. So those are some of the items and most of those that we mentioned before, a lot of that stuff will be able to fit into one M16 magazine pouch. Another thing that what we can do is to get some of your gallon size Ziploc bags, put in one of your bags of solution, one of your tubing, two of your needles, some alcohol and some betadine prep pads, gloves and some tape. Just put that in there and make that as one kit. And then if people go out and they start IV and they need to refresh that, they can drop that Ziploc bag, Italian aid station, you can give them a new one, you can regenerate the other kit back in there. And then it's good to go because obviously you know you're not going to use a whole roll of tape, something like that or you know if you only used one of the needles and things like that. is an option, but everybody needs to carry that. You don't need to load the medic down with the bags of IV solutions and things like that. Everybody needs to have one. I was going to say, the idea is to make sure that we break down the weight. That is the most critical issue here. Everybody carrying something, it's just like when airborne units issue out mortar rounds and machine gun ammunition for the squad gunner. It's not because they got the weapon, it's because he can't carry it all, right Mike? And also, what happens if you get separated or caught off and make it to where the medic is?
