I had a dream the other night that, well, I didn't understand. A figure walked in through the mist with a flintlock in his hand. His clothes were torn and dirty as he stood there by my bed. He took off his three-cornered hat, and speaking low to me, he said, we've fought a revolution to secure our liberty. We wrote the Constitution as a shield from tyranny. For future generations, this legacy we gave. In this, the land of the free. and home of the brave. The freedoms we secured for you we hoped you'd always keep. The tyrants labored endlessly while your parents were asleep. Your freedom's gone, your courage lost, you're no more than a slave. In this the land of the free and home of the brave. You buy permits to travel and permits to own a gun. Permits to start a business or to build a place for one. On land that you believe you own, you pay a yearly rent. Although you have no voice in saying how the money's spent, your children must attend a school that doesn't educate, and your Christian values can't be taught according to the state. You read about the current news in a regulated press, and you pay a tax you do not owe to please the IRS. Your money is no longer made of silver nor of gold. You trade your wealth for paper so your life can be controlled. You pay for crimes that make our nation turn from God and shame. You've taken Satan's number. You've traded in your name. You've given government control to those who do you harm so they could burn down churches and seize the family farm. And keep our country deep in debt. Put men of God in jail. Harash your fellow countrymen while corrupted courts prevail. Your public servants don't uphold the solemn oaths they've sworn. and your daughters visit doctors so their children will be born. Your leaders send artillery and guns to foreign shores and send your sons to slaughter fighting other people's wars. Can you regain the freedoms for which we fought and died? Or don't you have the courage or the faith to stand with pride? And are there no more values for which you'll fight to save? Or do you wish your children to live in fear and be a slave? Oh, sons of the Republic, arise, take a stand, defend the Constitution, the Supreme Law of the land, preserve our great Republic and each God given right, and pray to God to keep the torch of freedom bright. Burns, how to deal with burn casualties. Very, very important to know. For any of you that's had a burn, even minor burns, you'll know that that is probably the most painful you can have. And they're cycling them through on a regular basis. And a number of these are nonviolent criminals, or like in Mark's case, it was nonviolent, but they convicted him of a violent crime anyway. It was nonviolent by Mark, but it was violent to Mark. Right, but they convicted him of the violence. Which is, you know, and the whole, I'm sorry. aggravates me to no end and that is the idea that a prosecutor, the judge, and prosecutor, judge, and law enforcement, they can perjure themselves all day in court as long as they get a conviction. I want to see you mad, you know, get me talking about that for a while. I'm a first-hand witness to that too myself. Yeah. Well, I couldn't understand. I could not understand why on earth they were documenting their why right in front of me. And he did it. And I was just blown away. Do you not think I'm not going to fight? So be aware, you know, these people, they don't, they claim that they are They're diverse and they, everyone for who they are and what their beliefs are. Mike, you were just getting into how to treat burns. Yes. Can we continue please? Okay, very good. I just want to say this right up front. My name is Michael Nasser and I approve of this message. You know, we've all heard of the Hegelian dialectic thesis, anti-thesis, synthesis. There's something that can be referred to as problem-reaction solution. I was letting my mind wander the other day, and I made a comparison between radio programs that kind of fall into that category. You have the programs that talk about them. You have radio programs that talk about the react... But the intelligence report is the solution. If you want to know... what to do if you're tired of complaining about the problem and your radio or the TV, then that gives you... We're talking about... We kind of touched on this yesterday, but we'll kind of review back over that. So my main concern is to relieve pain. And I know a lot of people don't happen to have any of it handy if my dogs cough to give usually between eight and 10 milligrams of morphine, isosthenic light, light, Hold on there. Go ahead and take over for a second. Well, the lidocaine. Lidocaine, if you've gone and looked at sunburn medication on the shelf for pain, I think the best that I've found and I've won the burns easily is the lidocaine aloe vera mix, which I lost. I mean, it takes the sting away right now. It kills the pain. That is going to be used to put that pain. The other thing is, you know, for something that is immediate, you're going to want to cool that burn as fast as you can. Because the longer it sets, the more damage is done. I know he'll be dead here shortly, but I don't have, unfortunately, more of what he was talking about here. He is the expert, not me. Though I had dealt with some burns in the past, it's... I'm certainly no expert, but lidocaine, absolutely. And like I said, the other thing you want to do is get it cooled off as fast as possible. Okay, I'm back. Yes, go ahead. I don't know what was going on there. They were all laying down here fine. All of a sudden, they just sent back knowing. Anyway, I heard you talking about the lidocaine. Yes, I've also seen that even in somebody that had their face weren't pretty bad, they would just infiltrate the lidocaine. hypodermic made right under the skin and then they just went at them with the scrub picked then and there and it was pretty extensive burn. It was when the skin was hanging like it treated. I was more concerned that they might have inhaled throughout in the field in Saudi Arabia and they had to be a person. You're trying to light it but that's obviously what they did. They turned on the gas, they bent over the light the stove. The acid accumulated right there into the corner. I mean it was all in the face. They had instantaneous blisters forming on their, from the skin hanging off in sheets and everything. We treated them fast, got the IV in there, evacuated them to MC and the next thing they knew, they were on a plane ride to Germany. They were returned back to duty within, oh, I don't know, about a month or so and absolutely no scars, no nothing, no this, no that. Well, that's amazing. It was a good story there. Anyway. So yeah, if you happen to have it, morphine is good, the IV, the HN, MAM, IV. Also, you can infiltrate lidocaine directly in the area, and it needs, don't be all gingerly. I mind that the lidocaine is the area. You're going to use lidocaine, make sure it doesn't have any epinephrine, especially if you're going to be injecting it around the hands or anything, because the epinephrine is a preservative in the lidocaine, and it's going to cause the The nasal constriction is going to cause the vessels to shrill into the hands, into the small, these venules and everything. And then you're going to have a long-term problem. You could actually have the hand or the foot amputated or stuff that you're using. It was in the shock and go, aha, you're in shock. What do we do now? You need to treat not there. We want to make sure that we're going to prevent infection. Now, if the might have to be CPR and the beat in their breathing and stuff. That's going to take them again now you're going to remove all the clothing except scrub it pretty vigorously and you're going to be treating chemical burns and chemicals that react with water. Pull treatment of a chemical burn. Again it's going to vary with type of an acid burn. We can neutralize it. The washing area will carbonate. We can use a feedings so the ones that they try to neutralize those up to five and nine-ish or so on the pH scale continuing to burn there. Good examination of the patient. And this is where we've talked about the subject type of the patient. So we want to make sure that we have a good examination of the patient. The real careful attention to evaluating, we can get a good diagnosis. So we did history, the burning agent. And then if possible, if we can have them make an estimation of the duration of an examination to find out how much the about the difference on wtprn.com go to the archives. The next thing is you can the prognostic viewpoint to show a lot of less activity as a pin prick of using a pin or a dermic needle. If you happen to have some sterile tongue depressors, instead of breaking it off straight across, kind of do is kind kind of a diagonal. And then two, the bowl is a hypodermic needle and it's a dermic needle into a sharps container and all the... Also another method we can try to pull out one of the hairs of the skin. If the skin, the hair is not all burned away, if you can pull out the hair with little or no resistance or pain, and then we can be pretty reassured that the burn is pretty deep. Maybe he's listening in and seeing. Okay, so anyway, now we were going to talk about the calculation of the burn area itself. Now, you know, we're all familiar with the, you know, it's a percentage here and it's nine here and it's the front and it's the back and the one percent over here. And, you know, pretty soon you're going to have to get a calculator out to try to calculate all this. So this is where we're going with this new rule of points. Now, that's why I had you make your little stick man. At the beginning of number one is the head. Number two is the upper chest. Number three is the abdomen. Number four is the abdomen. And number five is the extremity. Each zone is going to be 20 per sequence. 100 makes it real easy. You can do this in your head. Now the front is going to be 10% will be 10% and that equals a 20%. All to this rule is going to be the number one zone. If somebody has been burned on their face and their face And then no matter if it's just the thrust, the back, it is still going to be categorized back, then it's going to be 20%. That's what we're doing with that. Just to try to make it easier. So, you know, what's the rule of nines, what's the rule of 10s, what's the one. So now, you know, so each one of those zones is 20%, the exception of the head, 1% for the back. Now, this is other than, like I said, the head, which we're, no matter what part of it is, we're still going to count that as 20. Now we're going to maybe class some of these third degree burns over an area of about third degree of the hands. And if there's any other burns complicating the respiratory injuries or if there's other major soft injury, all those will add into the critical category. Hands, the feet, or the face. And that's any one of those three. That doesn't mean the hands, the feet, and the hands. That would be that. And then if there's any other, if there's the burns and then they're complicated by respiratory major sauce, that would be a second between five and 10%. Again, not involving the hands, the feet or the face, involving again, the hands, the feet or the general rules of the treatment. Again, now we kind of touched that on that earlier with the infiltration of the lidocaine and the vigorous scrubbing. The cleaning of the burned area, it should be told, but pretty you need to have a, I keep wanting to say scratch pad, but that's really not a scratch pad to remove on skin and all the fili vigorously. And you also need to use a sterile net, and that's what needs to be done soon as possible. Burn is still net being pre-saved with just ordinary soap and tap water. But initially we want to make sure that we remove any contaminants. The next thing is to leave the ballistom tack After they've done that, then again, we've discussed this, you hear a great big whopping bang, kind of a small of an inch long, and they're done for real micro-flight. Then you want to also, them and the impact of any infection, usually the cleanest like water, and then look in there if it starts a pretty good sign, the littly that they'll swelling up too. If you determine that they'll go ahead and immediately go ahead and open them up and you breed it in a manner because now we've got sterile underneath that. We need to get a tetanus shot also in that. Now the ointments and everything, usually generally the rule of thumb is that initially we don't want to put any insulation layer. It's going to help pull that heat in there. We need to put in the wound that's into the thickness on top. It's going to make an insulating layer there. And then the skin is still going to have this heat underneath and it's still going to continue to cook underneath there. Be real, really loud. Any type of that, it all been cooled down on IV because it's likely going to have a lot of fluid lost. And there's a good possibility. And again, it depends on the place where the burn is, especially if somebody's been burned on the face. And another rule of thumb also when you're examining the The question you need to ask is, how to accumulate? Did it take 20 minutes for the blister to start? Because they're instantaneous. I would go ahead and start an IV. We can always pull it out later, especially with Serenity thing. And the blister, we can go ahead and we'll start the IV. And we have an open route, so then we can give them, if they start to go into shock or anything like that, then we have an open route already ahead. Like I had mentioned before, we treat them as if they are are in shock or not catch up. It's like, oh boy, we got to start an IV. Oh boy, we're in the crash cart. Oh, we got to have this.
