I had a dream the other night that, well, I didn't understand. A figure walked in through the mist with a flintlock in his hand. His clothes were torn and dirty as he stood there by my bed. The tyrants labored endlessly while your parents were asleep. Your freedom's gone, your courage lost, you're no more than a slave. In this the land of the free and home of the brave. You buy permits to travel and permits to own a gun. Permits to start a business or to build a place for one. On land that you believe you own, you pay a yearly rent. Although you have no voice in saying how the money's spent, your children must attend a school that doesn't educate, and your Christian values can't be taught according to the state. You read about the current news in a regulated press, and you pay a tax you do not owe to please the IRS. Your money is no longer made of silver nor of gold. You trade your wealth for paper so your life can be controlled. You pay for crimes that make our nation turn from God and shame. You've taken Satan's number. You've traded in your name. You've given government control to those who do you harm so they could burn down churches and seize the family farm and keep our country deep in debt. Put men of God in jail. Harash your fellow countrymen while corrupted courts prevail. Your public servants don't uphold the solemn oaths they've sworn. and your daughters visit doctors so their children will be born. Your leaders send artillery and guns to foreign shores and send your sons to slaughter fighting other people's wars. Can you regain the freedoms for which we fought and died? Or don't you have the courage or the faith to stand with pride? And are there no more values for which you'll fight to save? Or do you wish your children to live in fear and be a slave? And I understand we're having a little bit of trouble contacting Mark, so we're just going to kind of jump right into the program here, our usual format. And we're going to kick off with a little bit of human first aid. Now, the other day, we have just touched a little bit on tetanus. And we'll back over that. Now, remember our format of how we're taking notes here so we can break this down. So I want you to write tetanus, T-E-T-A-N-U-S. And now it's in a, and here's the definition of it. It's an acute central nervous system. We can abbreviate that by just writing CNS. It stands for central nervous system. Intoxication caused by toxins that are produced by the slender, positive anaerobic gastridia tetani. In plain language, that is a description of that type of period. It's slender, means it's, long and skinny. It's core forming. That means it's very hard to kill when it produces a very hard positive. That means that if you do a grabs, that's meaning the bacteria in the slide, positive, aerobic, and that means that it can serve without air, that it can survive internally as opposed to an aerobic that it has to live out in the open in the land. And then your basporidium is the botulinus or botulism poison. And there are now these spore-forming gram-positive bacteria found mainly in the soil and in the feces of animals and humans, and they enter the body by wound contamination, and they can also enter into a newborn infection often occurs through the umbilical stump, and your incubation period is between five and 15 days. Now that's the definition now on the side of your paper, where we're going to follow the SOAP format, S is an N, P. Now, next to A is these are going to be your subject findings. Now, occasionally your first symptom is going to be pain and tingling at the wound site, and then it's going to be fall of the city of the nearby muscle group. So it's going to start to chasm on you. Maybe all that can happen. Now, usually the presenting symptoms are stiffness of the next stiffness, call fee is suave and the irritability. And then as it progresses, then you're going to have a hyperlexation. It's going to develop later. That means the muscles are going to want to... If it's in your arms or your legs, you're going to bend it to me and pull out more than normally. Hyper means above as opposed to a hypodermic needle, which means below. And then what's going to happen is it progresses the spatial muscles and rigidity and spasm. The muscles are the abdomen and the neck. going to be as your progression of your signs and symptoms there. Now your objective findings on examining the patient, what you're going to find are painful, tonic convulsions and those are going to be called minor stimuli, especially if there's any loud noises or anything like that and those are very common. Now the patient is going to be awake and alert during the entire illness. Now during the convulgolitis, that's in the muscle that keeps you from going down the wrong pipe. The glottis and the respiratory muscles go into spasms so that the patient is unable to breathe and cyanosis and asphyxia are common. The temperature is lightly elevated, usually leukocytosis and the diagnosis of tetanus is made clinically. Case down under A, under our soap format is you this is where your diagnosis this is your appearance of what you looked at under the ass and the home and then you put it together and your assumption or your appearance is going to be tetanus there could possibly be differential diagnosis there could be of acute sense nervous the anacin fact and other types of nine poisoning and they should also be considered Now next to P, and this is going to be your plan of how you're going to treat the patient, immunization with tetanus toxoid should be universal. The debridement of the wound's immunization is most turbulent from human is that there's going to be 5,000 units given intramuscular. It's not available for the type of activity for the horse serum. They have 100,000 units in IV. and the anti-convulsant therapy should not be substituted as necessary. A tracheostomy, that's going to be a hole in the neck to help them breathe and or assisted rep rest may be required. You know, how am I going to be doing all these things out in the field? And you know, the main thing is, is what we're trying to do here is to, we talk about prevention a lot of times. And so we're going to try to make sure that we're you know, not walking around where we're going to be coming in contact with possibly contaminated soil, having any wounds on our feet or anything that we can do. And, you know, and I'm not suggesting everybody out there, but there's certain medical units and the people that they do have the ability and the knowledge that they can go ahead and enroll themselves in a community college microbiology, and then they'll teach you how to prepare stains and slides and do the examination and to figure them. So it might be worthwhile for your investment, college time, the tuition, the buy yourself a quality microscope, some slides, a loop, size, when you're doing the specimens, figure out how to make some colts. And it's kind of an elaborate job, but it's going to be worthwhile. It's going to pay off down the road. I don't expect everybody to do this. And it can be mystifying at first. especially when we're talking about forming gram-positive anaerobic vestridia and all these things that are out there. And another thing that what we're trying to do is to eliminate the unknown things that happen and that's how we're going to treat this. We don't even know what it is, let alone, and you know, I know I've been talking about nations and myrrh derivatives or they're in it and sort of viruses and stuff. I'm giving you the medical skinny as it is to be up to you. near your homemade remedies may be able to heal. They certainly have our grandparents back when they didn't have any antibiotics and all these other things. There's going to be a certain extent as to where medical intervention is going into treatment of these illnesses and diseases. Of course, as we all know, in times of a good healthy body in the beginning of the kickoff in October of about having, you know, intake of fluids and everything. strengthen your body and strengthen your immune system so you're going to be less to step together. Not only that, but then it's also the drain on the medical supplies and all the rest of these things they're associated with. Can you give me an idea for like a minimum or a first aid kit? Oh, absolutely. We'll kind of break it down into a couple of three different categories. One of them is going to be your, the little first aid, the compass, the placer, the nylon kit that oftentimes you'll find trumpets in. And then I would suggest either getting a large compress and then in also I've seen some of them that are similar sizes that they, it's a similar size to the, the, the compress, but in addition to that, then it can also a similar package for smaller costs in there. So that might be a, you can go with that one. And then also, I'd also recommend in that small first aid top is either your large compressor, the similar size packages with the four smaller ones, triangular bandage and some band aids. You put the band aids between the compress and the triangular bandage and put those in that small. And then I would have that down on my shelf. Don't wear it up on your shoulder strap like they do in all the movies. Because if you ever have you're firing your right your shoulder fired rifle If you ever have to switch sides and engage the enemy with your other shoulder then it's going to interfere with the one I would recommend is the snaps and it's got the Alice clips in the back and also in the center of it there's a connector a lot of times they come with a side that's hinged from that rigid class because what happens is when you start taking a few things out of it then everything else inside there the empty pill bottles and and everything, they're just rattling around. What I would recommend is getting a gallon size Zippom 2x2s, some iPad, cushiony, oh, an Elasti-style type gauze, package of sterile gloves, and sterile gloves comes in all different sizes. They'll be wearing, oh, about somewhere between a 7, 7 1 by 5 or an 8 or so. If you get a 2 size, if you wear them for a time, you're either some just plain examination gloves. You can usually buy a package of those at the dollar store. Get some molesk roll that you can cut off. Or maybe add in some two items in the bottom of the ziplock. Put half of them over in one side, half of them in the other side. Rev it out, evacuating all the excess air in there. And then put that inside that. And then what do you say, iris scissors? only about a half of an inch long you can use those for a hemostat. Or hemo usually used for if there's a vessel that you can reach in and clap as a vessel or cling and everything. And then what I'd also do is then get some small 50 peri individual strands out of the center. The ring where you put your fingers for the scissors and the hemostats then ring that through there and then have that tied on to where the individual first wind up the extra the side of a can-team cover, put one in a cloth off, guide the needle, there's really no way to clear that out. So, as always, there's a backup bill. And you want to have some iodine prep either your puncture site before you IV. And then you also need to have some alcohol after you've prepped iodine. And also, before you've given somebody iodine, you always want to make sure that they don't have any allergies, any known drug medication reaction type of thing. And then after you've prepped site iodine what happens is after you've started your IV and going to stick to the iodine on the skin so you need to strip that tourniquet. The tourniquet that could be improvised you can use your exam gloves as a tourniquet and other things like that. Okay, and we were talking about the recommended supplies for individual first aid kits and as some other recommendations that you might want to consider, I'd also add to that some ChapStick, some aspirin, acetaminophen or Tylenol. Okay. Aspirin? I'm right and I'm Drenrodes, yes I can. Okay. Acetaminophen, that's A-C-E-T-O-M-O-P-H-E-N. Acetaminophen, something like that. We usually call it Tylenol, but if you go by the Tylenol brand, you're paying for a trade name, but the plain Acetaminophen is. You can get some water purification tablets, some iodine tablets. You can get those at your camping store. I make sure that you get a bottle that's sealed up at the top. Usually they'll have a wax seal on there. And then you look at the tablets, make sure that they're not real old. They're good steel gray. They haven't started turning red or a humble in there. You need to get a three by five spiral notebook and roll one of these little pad things that you can carry in your pocket. And then I get a pencil, get a regular number two pencil, a puffy eraser and sharpen both ends. And that way then if you're writing and you make a mistake then you can just line that out. You don't need to erase it. And then if one of the ends breaks then you can just flip it over and continue to write. Often times if you have pens, depending on your area, if you're operating in cold weather or light-headed, an odd angle, work, and they always seem to put out any warning, you'll be writing along all over. The next step... And also, then I would also recommend on these things that we've just covered here in the small pouch and in the individual first and these other things, that they need to be freed by each injured. We want to use their medical supplies for using our medical supplies because when it is evacuated, they usually take their gear with them. Because your medical is down the road, then you're going to be out of first individual to have not going to be him down because he still has to carry the rest of his gear and everything like that. So this way we disperse. If anybody gets separate, divide the patrol in half, then they still have their own role and one takes the high road and one takes the low road there and go, well here, I'll give you these two by twos and some, and you've got some normal saline and all this stuff. And then if you'd highly recommend items, a couple of other stations got a little pis pouch and a canteen cover. medical bag that might be what medic is carrying. Now that's going to be a redundant of what we've already discussed here. Gloves, I would recommend having some sterile gloves individually wrapped sterile. You can search around. So look on the internet and stuff and you come to the, they're going to charge you about $7 for one pair of them to some of these websites. You can buy a whole box of them and they'll be shipped to your dollars and that's pairs of them. So now you've dropped it to be anything that's You can't have a sub-state, you can't have a field expedient. It's either it's sterile or it's not. So I'd highly recommend supplies. Full supplies, it seemed to me, I was over a million. It's on there to serve right to your door. For a time doing that, IVs, IV tubings and stuff from your vet, from your veterinary supply store, these other things that you can find on the internet, whether in the chapstick, Tylenol, the... You don't have to necessarily go down to the or drugstore and paid for the very same item. You might have to buy a dozen rolls of tape, rolls of tape down to the drugstore and buy one out. Here's the next list for the medic, in the medics aid bag. The exam gloves you can buy a whole box of. Sometimes we really don't need all in a bag where you can go down to the dollar store, two by twos, some two inch, three inch and four I-M-G. It's real nice. Used to be four by four. the last the stock had regular roller bars did on it really wouldn't come you're trying to found somebody's hand arm or something like that that has somebody to hold the end of the dressing on while you've got some paper something to to secure the end of it because if you let it go it would just start unwraping all by itself get some IV solutions and also I would recommend instead of getting the leader IV solutions to go ahead and get your five option whether it's normal saline, fingers lactate, that way then each person and you'll have a variation because there's certain blood transfusion we run on specific. Now you can just use regular hypodermic needles. The gel colons with the military surplus, or I'd recommend getting a minor circuit. They usually run about 20, 30 with the Canvas kit. Have your hemostat and a needle hole, the knife handle, the scalpel blade, a small pair of scissors, the sutures. the probe and then your suture scissors and those are all specific things. Now the difference between a hemostat and a needle holder. It looks kind of like a pair of scissors, so to say. You've got the place down at the bottom of the two loops where you put your... and then it goes up to where the jaws are. And then there's also a ratchet down in there that you can clamp together in it. Open that up and look at the... because a hemostat will have lines just running horizontally. All the diamond pattern comes out. The skin... or you grasp the skin on the other side when your hemostat... or excuse me, the suit... other side and then tie it off. It's a normal hemostat when you would go to poke. I'd recommend going to the military surplus stores or going to a gun show or something like that and look at it. It has a lot of medical supplies and things. Good parisers the other day and they only cost me 50. They usually just throw away all these instruments when they're done. Thing is that I'd recommend is having some Stary strips. They come in different little pieces of minor wound iodine or betadine is just a different name of it. One of them I take the two inch tape or anything else. You can make a half an inch one or anything off of your two inch, but you can't make the flashlight. And don't get one of those just those cheapy disposal eye exam type flashlights. Go ahead and get something like a small mag light where you can replace the batteries and get the rechargeable batteries for your hand aids. A skin. pill bottles, cementi pill bottles. So in case anybody's got a headache or something like that or a fever, then you can dispense some of these medications and then you can put them in because if you have a bottle of say, you know, a hundred Tylenols or something, you're not going to give that person the whole bottle. You're going to just give them a couple of them and then you're not going to just want to put it into their pocket or anything like that. Then you have the cementi pill bottles there. Get some hypodermic needles, those real handy and if you examine the tipo, not only it's cut on an angle, but around the point of it, it's opened on there so you can actually move sliver of wood or metal in your finger. Some of that newer stuff that they're using over in Iraq to cut off wounds and stuff, I hear it's very effective, but I haven't ever used any of that, no sir. Okay. You mentioned the tourniquet. There are ones that are made up. Is it worth it to get one of those or just do it with 550-1,000-wood? Well, you can use one of your exam gloves. Just stretch that out. Have one hand on the cuff and the other hand down by the fingers and just stretch it like if you're going to start an IV up in somebody's anti-cubital area by their elbow. You can just use the exam glove there. Just wrap it around and tuck it in there and that'll serve as a tourniquet. How about the battle dressing? There's some bigger ones called the Israeli Bandage. Have you any experience with them? I've seen some of those when I was over in Iraq. I also got some from the Iraqi army. They're pretty gigantic though. And I guess it would be handy to maybe have one or two of those. But the only problem is they just take up so much space. At one point, people were, it's been years now, but people were talking about using DMSO for arthritis and stuff like that. I understand it promotes interstitial transport of medication so you could use that arthritic pain or if you wanted to make up a solution of something that you can add the DMSO in there and if you're going to put it on topically then that's going to help promote the transport of that medication into and in through the skin and it makes that more effective. That was what I wanted to know. If you could go on with the Aedman's kit please. Okay, sure can. Okay, so we stopped the needle. I did some water purification tablets and K-opecta, stocks diarrhea. I get some Neosporin ointment or some they call it triple antibiotic ointment. Some cotton tip applicators or Q-tips or so. The individual ones are a little bit better, especially if we're going to be applying some ointment or stuff because if you just have regular Q-tips out of a box. They're sterile, but they can get dusty after you've opened the box or so. So you might want to have a little bit of both. Have just a small box of Q-tips or take some out of a bigger box and put them into a Ziploc bag and close those. But I'd also try to get some tip applicator. I get a tongue depressor, three of those. Again, then I get another three by five spiral notebook. and with our pencil arrangement where you break off the eraser, start both ends, and then we can stick the pencil up and through the spiral part so it's contained there right in that notebook. I'd have a poncho, and you don't have to get a real fancy Littman cardiology too and spend $200. Sometimes these little ones work just fine for that and everything. I recommend getting a copy of the original US Army Special medical handbook and its name is ST31-91B. It's about the size of a paperback book. They do have a newer one, it's much larger but they've gotten away from a lot of the primitive medicine and the veterinary medicine in the newer ones.
